摘要
目的 比较不同囊胚培养策略在反复种植失败(RIF)患者中的应用效果,为使RIF患者获得临床妊娠的机会,提供更好的措施.方法 回顾性分析2011年8月至2016年6月苏北人民医院生殖中心收治的249例RIF患者,按照囊胚培养策略分为新鲜囊胚培养组(166个周期)与冻胚解冻囊胚培养组(93个周期).比较两组胚胎形成情况、胚胎质量及妊娠结局.结果 冻胚解冻囊胚培养组的囊胚形成率、可移植囊胚形成率以及优质囊胚形成率与新鲜囊胚培养组比较差异未见统计学意义(P>0.05).冻胚解冻囊胚培养组的临床妊娠率以及胚胎种植率高于新鲜囊胚培养组(P>0.05),但两组临床妊娠率比较差异未见统计学意义(χ2=1.617,P>0.05),种植率比较差异有统计学意义(χ2=9.525,P<0.01),流产率、异位妊娠率、持续妊娠率比较差异未见统计学意义(P>0.05).结论 新鲜囊胚培养或是冻胚解冻囊胚培养策略的选择,不会改变囊胚形成的效率和质量,也不改变移植后的妊娠结局.
Objective To compare the effects of different blastocyst culture strategies on patients with repeated implantation failure(RIF),and to provide better measures for RIF patients to get clinical pregnancy opportunities.Methods Retrospective analysis was performed on the results of the fresh blastocyst culture and blastocyst culture after frozen-thawed cleavage stage embryo in 249 RIF patients from August 2011 to June 2016 in Reproductive Medical Centre of Northern Jiangsu People's Hospital.Patients were divided into two group according to blastocyst culture strategies,fresh blastcyst culture group(166 cycles)and frozen-thawed cleavage stage embryo culture group(93 cycles).Results There was no significant difference in blastocysts formation rate,transplantable blastocysts formation rate or high quality blastocysts formation rate between fresh blastocyst culture group and frozen-thawed cleavage stage embryo culture group(P>0.05).Clinical pregnancy rate and embryo implantation rate of frozen-thawed cleavage stage embryo culture group were higher than those of fresh blastocyst culture(P>0.05),but there was no significant difference in clinical pregnancy rate between the two groups(χ2=1.617,P>0.05);three was significant difference in embryo implantion rate between the two groups(χ2=9.525,P<0.01).And no significant difference was noted in abortion rate,ectopic pregnancy rate and persistent pregnancy rate between the two groups(P>0.05).Conclusions Fresh blastocyst culture has similar efficacy to frozen-thawed cleavage stage embryo strategy.They can culture embryo of same quality,and has similar pregnancy outcome after transplantation.
