摘要
目的 通过探讨不同年龄段高龄经产妇的妊娠特点及结局,加强提高对高龄经产妇分娩的认识以及具体管理.方法 选取2017年1月1日至12月31日期间分娩的经产妇共2815例? 其中分娩年龄≥40岁112例(高龄1组),35~39岁898例(高龄2组),<35岁(适龄组)1805例.比较分析三组产妇的一般临床资料、妊娠特点及妊娠结局.结果 三组产妇孕前BMI及本次妊娠距离前次分娩时间分别比较,差异均有统计学意义(P均<0.05);孕次比较,高龄1组与高龄2组均高于适龄组(P<0.05),高龄1组与高龄2组比较,差异无统计学意义(P>0.05).妊娠期高血压疾病、妊娠期糖尿病、前置胎盘、胎盘早剥、产后出血的发生率分别比较,差异均有统计学意义(P均<0.05),胎膜早破发生率分别比较,差异无统计学意义(P>0.05).三组产妇中,早产儿、巨大儿、低出生质量儿的发生率分别比较,差异均有统计学意义(P均<0.05);新生儿窒息发生率比较,差异无统计学意义(P>0.05).三组产妇中,剖宫产率分别比较,差异均有统计学意义(P均<0.05);阴道助产率分别比较,差异无统计学意义(P>0.05).结论 高龄经产妇妊娠合并症及并发症风险与年龄增呈正相关,对于妊娠结局及新生儿的影响较大,临床上应加强高龄经产妇的围生期和分娩期管理,保障母婴安全.
Objective To explore the characteristics and outcomes of maternal pregnancy in different age groups,and to strengthen the understanding and management of maternal delivery.Methods A total of 2815 multiparous women were selected from January 1 to December 31,2017.Among them,112 parturients aged over 40(group 1),898 parturients aged 35 to 39(group 2)and 1805 parturients aged less than 35(age-appropriate group).The general clinical data,pregnancy characteristics and pregnancy outcome of the three groups were compared and analyzed.Results BMI before pregnancy and the time between this pregnancy and the previous delivery were compared among the three groups,and the differences were statistically significant(all P<0.05).Pregnancy time comparison showed that both group 1 and group 2 were higher than that of the age-appropriate group(P<0.05),while there was no statistically significant difference between group 1 and group 2(P>0.05).The incidence of gestational hypertension,gestational diabetes mellitus,placenta previa,placental abruption,and postpartum hemorrhage were compared,and the differences were statistically significant(all P<0.05),and the differences were not statistically significant(P>0.05).The incidence rates of premature infants,macrosomia infants and low-birth quality infants in the three groups were compared,and the differences were statistically significant(all P<0.05).There was no significant difference in the incidence of neonatal asphyxia(P>0.05).Cesarean section rates were compared among the three groups,and the differences were statistically significant(all P<0.05).There was no significant difference in vaginal midwifery rate(P>0.05).Conclusion The risk of gestational complications and complications in elderly women is positively correlated with age,and has a greater impact on pregnancy outcomes and neonates.It is necessary to strengthen the management of perinatal and delivery during the period of maternal age and to ensure maternal and child safety.
