摘要
目的 探讨美兰染色联合乳腺X线立体定位在乳腺不可触及病灶活检中的应用.方法 回顾性分析2016年1月至2018年4月间收治的93例经乳腺钼靶摄片提示钙化灶、乳腺影像报告和数据系统分级为4~5级且临床触诊及超声检查阴性患者的临床资料.93例患者均行乳腺X线立体定位导丝引导下的活检,其中51例行美兰染色联合乳腺X线立体定位引导活检,设为观察组;42例行乳腺X线立体定位引导活检,设为对照组;比较两组一次性切除目标病灶成功率及切除的标本重量.结果 93例患者中85例一次性成功切除目标病灶,8例≥两次切除病灶;观察组51例均一次性切除病灶,切除的标本平均重量为(39.96±3.53)g;对照组34例一次性切除病灶,8例≥两次切除病灶,切除的标本平均重量为(49.07±7.08)g.两组定位后一次性切除目标病灶的成功率及切除的标本重量比较均有统计学差异(P<0.05).结论 美兰染色联合乳腺X线立体定位较单用乳腺X线立体定位一次性切除病灶成功率高,创伤小,且操作简便,值得临床推广.
Objective To discuss the application of methylene blue staining combined with mammography in biopsy of non palpable breast lesions.Methods The clinical data of 93 cases of calcification with breast imaging reports and data systems classified as 4~5 grade,clinical palpation and ultrasound examination negative in Zhejiang Lishui Central Hospital from January 2016 to April 2018 were retrospectively analyzed.All 93 cases were performed biopsy under the mammography guided,of which 51 patients underwent methylene blue staining combined with breast X-ray stereotactic guided biopsy as observation group,42 patients underwent breast X-ray stereotactic guided biopsy as control group,and the success rate of one-time resection of the target lesion and the weight of the resected specimens were compared between the two groups.Results Of the 93 patients,85 were successfully resected once,8 twice or more;51 patients in the observation group were resected once,the average weight of the resected specimens was(39.96±3.53);34 patients in the control group were resected once,8 patients were resected twice or more,the average weight of the resected specimens was(49.07±7.08).There were significant differences in the success rate and the weight of the resected specimens between the two groups(P>0.05).Conclusion Compared with mammography stereotactic localization,methylene blue staining combined with mammography stereotactic localization has higher success rate,less trauma,simple operation,and is worthy of clinical promotion.
