摘要
目的 探讨胰头部肿瘤局部切除术的体会.方法 回顾性分析20例胰头部肿瘤局部切除术患者的资料.结果 本组患者均成功行局部切除术.无严重并发症病例,7例患者出现胰瘘,发生率为35%,其中A级胰瘘5例,B级2例.1例胰瘘后腹腔出血.均经保守治疗后痊愈.结论 对于胰头部良性或低度恶性肿瘤,保留器官功能的局部切除术是合理的术式,但需术者具备丰富的手术经验.胰瘘及出血是主要并发症.
Objective To share experience of performing local resection for pancreatic head tumor.Methods Data of 20 patients with local resection were collected and retrospectively analyzed.Results All of patients with pancreatic head tumor received successful local resection.There were no serious complications.But,7 patients presented pancreatic fistula with an incidence rate of 35%,consisting of 5 cases with grade A pancreatic fistula and 2 cases with grade B pancreatic fistula.Intraperitoneal hemorrhage after pancreatic fistula was observed in 1 case.All cases were cured after conservative treatment.Conclusion For the benign or low-grade malignant tumor in the pancreatic head,the local resection is a reasonable operation method for maintaining the pancreas function,but the surgeon needs rich experience.Pancreatic fistula and hemorrhage are the main complications.
