摘要
目的 采用静息态功能磁共振成像技术(rs-fMRI)利用低频振幅(ALFF)及基于种子点的功能连接(FC)方法探讨无先兆偏头痛患者发作间期大脑皮层活动性及脑区间功能连接的变化及其发病机制.方法 对无先兆偏头痛患者24例与性别、年龄相匹配的24例健康志愿者行静息态功能磁共振扫描,应用低频振幅方法分析两组受试者大脑活性的改变;利用基于种子点的功能连接方法以中脑导水管周围灰质(PAG)作为种子点的感兴趣脑区,进行全脑功能连接分析.结果 与健康受试者比较,偏头痛患者两侧枕中回、左侧距状回、两侧小脑、两侧颞下回、左侧颞中回、右侧梭状回、右侧枕上回低频振幅值显著降低;右侧楔前叶、左侧额中回低频振幅值显著增高;以中脑导水管周围灰质作为种子点,未发现有与其功能连接增高或减弱的脑区.结论 静息状态下偏头痛患者多个功能脑区存在异常激活及抑制,其中皮层扩散抑制(CSD)起始的脑区活动性降低,这可能与无先兆偏头痛的发病有关.
Objective To investigate the underlying mechanism of migraine without aura by exploring the changes of cerebral cortex activity and functional connectivity(FC)on resting state functional magnetic resonance imaging(rs-fMRI).Methods Interictal migraine without aura patients(n=24)and genders and age matched healthy subjects(n=24)were recruited.Gradient-echo EPI sequence was performed in all subjects.Amplitude of low-frequency fluctuation(ALFF)and seed-based in periaqueductal grey functional connectivity(FC)were employed to observe the changes of cerebral cortex activity and full brain functional connectivity.Result Compared with the control group,ALFF values in migraine without aura group had a significant decrease in the bilateral middle occipital gyrus,left calcarine,bilateral cerebellum,bilateral inferior temporal gyrus,left middle temporal gyrus,right fusiform gyrus and right superior occipital gyrus.Increased ALFF values were found in the right precuneus and left middle frontal gyrus.No brain area was found have functional connectivity with the periaqueductal grey(PAG).Conclusion Our findings suggest that cortical spreading depression may be important in the pathophysiology mechanism of migraine without aura,and some brain regions in the migraineur demonstrate abnormal in the resting state fMRI.
