西替利嗪和布地奈德联合治疗小儿哮喘的疗效观察 被引量:7

摘要 目的:探讨西替利嗪与布地奈德联合治疗小儿哮喘的疗效及对患儿肺功能、症状评分、炎症反应的影响。方法:收集2015年2月—2017年1月我院收治的哮喘患儿160例为研究对象,按照随机数字表法分为试验组和对照组,均80例。对照组给予布地奈德雾化吸入(100μg,bid)治疗,试验组在对照组基础上联合西替利嗪(1 mL,qd)治疗。比较两组临床症状评分、肺功能、炎症因子水平及不良反应发生情况。结果:治疗后两组日间评分和夜间评分均较治疗前明显降低,且试验组显著低于对照组,差异有统计学意义(P <0.01);治疗后两组用力肺活量(FVC)、第1 s呼气容积FEV1/FVC(FEV1%)、以及呼气高峰流量(PEFR)水平均较治疗前显著升高,且试验组显著高于对照组,差异有统计学意义(P <0.01);治疗后两组肿瘤坏死因子(TNF)-α,白细胞介素(IL)-6以及IL-10水平均较治疗前明显降低,且试验组显著低于对照组,差异有统计学意义(P <0.05或P <0.01);两组不良反应比较,差异无统计学意义(χ~2=1.060,P=0.303)。结论:布地奈德雾化吸入联合西替利嗪可有效改善哮喘患儿临床表现,减轻炎症反应,有助于肺功能恢复,且安全可靠。 Objective:To investigate the efficacy of cetirizine combined with budesonide in the treatment of asthma in children and its effects on lung functions,symptom scores and inflammatory response.Methods:160 children with asthma admitted to our hospital from February 2015 to January 2017 were enrolled as subjects investigated and randomly divided into an experimental group and a control group,80 cases each.The children in the control group were given budesonide inhalant(100μg,bid)treatment,and the children in the experimental group were given cetirizine(1 mL,qd)treatment on the basis of the control group.The clinical symptom scores,lung functions,inflammatory factor levels and adverse reactions were compared between the two groups.Results:The daytime scores and nighttime scores in the two groups after treatment were significantly lower than those before treatment,and the scores in the experimental group were significantly lower than those in the control group,with statistically significant differences(P<0.01).The forced vital capacity(FVC),forced expiratory volume in 1 second FEV1/FVC(FEV1%)and peak expiratory flow rate(PEFR)in the two groups after treatment were significantly higher than those before treatment,and these indexes in the experimental group were significantly higher than those in the control group,with statistically significant differences(P<0.01).The levels of tumor necrosis factor(TNF)-α,interleukin(IL)-6 and IL-10 in the two groups after treatment were significantly lower than those before treatment,and the levels in the experimental group were significantly lower than those in the control group,with statistically significant differences(P<0.05 or P<0.01).There was no statistically significant difference in adverse reactions between the two groups(χ2=1.060,P=0.303).Conclusion:Budesonide aerosol inhalation combined with cetirizine can effectively improve the clinical manifestations of asthmatic children,reduce inflammation response and help to remove lung functions,and is safe and reliable.
