股骨近端防旋髓内钉治疗老年股骨粗隆间骨折的临床疗效 被引量:10

Proximal Femoral Nails Antirotation in the Treatment of Femoral Intertrochanteric Fracture in Older Patients

摘要 目的 探讨应用股骨近端防旋髓内钉 ( PFNA) 治疗老年股骨粗隆间骨折的疗效及对机体关节系统功能带来的影响.方法 选取2014 年2 月~2017 年9 月入住我院骨科的老年股骨粗隆间骨折患者97 例, 其中50 例施予PFNA疗法为PFNA组, 47 例施予动力髋螺钉疗法 ( DHS) 为DHS组.统计两组病例优良率, 同时对其手术指标、关节功能临床改善水平作综合记录与测定.结果 术后PFNA组优良率94.00% , 高于DHS组78.72% ( P<0.05);PFNA组引流量、手术耗时、失血量显著低于 DHS 组 ( P<0.01);随访至今, PFNA组髋关节功能计分高于DHS组 ( P<0.05) .结论 对于老年股骨粗隆间骨折病例, PFNA疗法治疗更具可靠性, 对于机体关节功能改善具有促进作用, 推荐骨科视作优选疗法. Objective To investigate the effects of proximal femoral nails antirotation(PFNA)in the treatment of intertro-chanteric fractures in older patients and the effects on the joint function.Methods 97 older patients with intertrochanteric fractures from February 2014 to September 2017 were enrolled.Among them,50 patients receiving PFNA therapy were selected as PFNA group and 47 patients receiving dynamic hip screw therapy(DHS)were selected as DHS group.The excellent rates in two groups were counted.The surgical indicators and joint function were recorded and measured.Results The excellent rate in PFNA group(94.00%)was higher than that in DHS group(78.72%)(P<0.05).The drainage volume and blood loss were significantly low-er,and the operation time was significantly shorter in PFNA group than those in DHS group(P<0.01).The score of hip function in PFNA group was higher than that in DHS group(P<0.05).Conclusion PFNA therapy is more reliable and effective in the treat-ment of intertrochanteric fractures in older patients.It can promote the improvement of joint function.
