摘要
目的分析多层螺旋CT(MSCT)灌注参数与直肠癌分化程度、临床病理分期的关联性。方法抽取南阳市第一人民医院2020年2月至2022年1月收治的直肠癌患者65例作为研究对象。所有研究对象均行MSCT检查。比较直肠癌患者直肠癌组织和正常直肠组织MSCT灌注参数[血容量(BV)、血流量(BF)、平均通过时间(MTT)、血管表面通透性(PS)、达峰时间(TTP)];比较直肠癌患者不同分化程度、不同临床病理分期MSCT灌注参数;分析MSCT灌注参数和分化程度、临床病理分期的相关性。结果直肠癌组织BV、BF、PS、TTP参数高于正常直肠组织,MTT参数低于正常直肠组织(P<0.05);低分化患者BV、BF、PS、TTP参数高于中、高分化患者,MTT参数低于中、高分化患者(P<0.05);Ⅲ~Ⅳ期患者BV、BF、PS、TTP参数高于Ⅰ~Ⅱ期患者,MTT参数低于Ⅰ~Ⅱ期患者(P<0.05);Pearson分析结果显示,BV(r=-0.50)、BF(r=-0.51)、PS(r=-0.62)、TTP(r=-0.44)和分化程度呈负相关,MTT(r=0.61)和分化程度呈正相关(P<0.05);BV(r=0.51)、BF(r=0.61)、PS(r=0.69)、TTP(r=0.65)和临床病理分期呈正相关,MTT(r=-0.536)和临床病理分期呈负相关(P<0.05)。结论MSCT灌注参数和直肠癌分化程度、临床病理分期密切相关,可为临床诊断提供科学依据。
Objective To analyze the correlation of multi-slice spiral computed tomography(MSCT)perfusion parameters with the degree of differentiation and clinicopathological classification of rectal cancer.Methods Sixty-five patients with rectal cancer treated in Nanyang First People’s Hospital from February 2020 to January 2022 were selected as the research subjects.All patients underwent MSCT examination.MSCT perfusion parameters,including blood volume(BV),blood flow(BF),mean transit time(MTT),permeability surface(PS)and time to peak(TTP),of rectal cancer tissue and normal rectal tissues in rectal cancer patients were compared.The MSCT perfusion parameters of rectal cancer patients with different degrees of differentiation and different clinicopathological classifications were compared.The correlation of MSCT perfusion parameters with differentiation degree and clinicopathological classification was analyzed.Results The parameters,including BV,BF,PS and TTP,in rectal cancer tissues were higher than those in normal rectal tissues,while MTT in rectal cancer tissues was lower than that in normal rectal tissues(P<0.05).The BV,BF,PS and TTP in the poorly differentiated patients were higher than those in the moderately and well differentiated patients,and the MTT was lower than that in the moderately and well differentiated patients(P<0.05).The BV,BF,PS and TTP in patients at stage III-IV were higher than those in patients at stage I-II,and the MTT was lower than that in patients at stage I-II(P<0.05).Results of Pearson analysis showed that BV(r=-0.50),BF(r=-0.51),PS(r=-0.62)and TTP(r=-0.44)were negatively correlated with the degree of differentiation,while MTT(r=0.61)was positively correlated with the degree of differentiation(P<0.05);BV(r=0.51),BF(r=0.61),PS(r=0.69)and TTP(r=0.65)were positively correlated with clinicopathological classification,while MTT(r=-0.54)was negatively correlated with clinicopathological classification(P<0.05).Conclusions MSCT perfusion parameters are closely related to the degree of differentiation and clinicopathological classification of rectal cancer,which can provide a scientific basis for clinical diagnosis.
