摘要
目的探讨相对微创有限切开髂腹股沟入路重建钢板内固定(RPIF)治疗髋臼骨折的临床效果。方法抽取2017年9月至2021年9月郑州大学第二附属医院收治的40例髋臼骨折患者为研究对象,患者均接受RPIF治疗,根据入路方式分为对照组与观察组,每组20例。对照组采用常规髂腹股沟入路,观察组采取相对微创有限切开髂腹股沟入路。比较两组的疗效、髋关节功能、手术出血量、术后疼痛评分、骨折愈合时间及并发症。结果观察组髋臼骨折复位优良率(90.00%,18/20)及髋关节功能优良率(90.00%,18/20)均高于对照组(65.00%,13/20;65.00%,13/20),差异有统计学意义(P<0.05)。观察组手术出血量少于对照组,术后疼痛评分低于对照组,差异有统计学意义(P<0.05);两组骨折愈合时间比较差异未见统计学意义(P>0.05)。观察组并发症发生率(10.00%,2/20)低于对照组(40.00%,8/20),P<0.05。结论相对微创有限切开髂腹股沟入路RPIF治疗髋臼骨折,不仅复位效果好,髋关节功能恢复好,而且手术出血量少,术后疼痛更轻,并发症发生率低。
Objective To investigate the effect of reconstructive plate and internal fixation(RPIF)via ilioinguinal approach with relatively minimally invasive limited incision in the treatment of acetabular fractures.Methods A total of 40 patients with acetabular fractures treated in the Second Affiliated Hospital of Zhengzhou University from September 2017 to September 2021 were selected as research objects.All of them were treated by RPIF,and they were divided into control group and observation group according to surgery approach,with 20 cases in each group.The control group underwent surgery via conventional ilioinguinal approach,and the observation group underwent surgery via ilioinguinal approach with relatively minimally invasive limited incision.The efficacy,hip joint function,amount of operation blood loss,postoperative pain scores,fracture healing time and complication were compared between the two group.Results The excellent and good rate of acetabular fracture reduction and hip joint function in the observation group(90.00%,18/20;90.00%,18/20)were higher than those in the control group(65.00%,13/20;65.00%,13/20),P<0.05.The amount of operation blood loss and postoperative pain scores of the observation group were lower than those of the control group(P<0.05).There was no significant difference in fracture healing time between the two groups(P>0.05).The incidence of complication of the observation group(10.00%,2/20)was lower than that of the control group(40.00%,8/20),P<0.05.Conclusions Performing RPIF via ilioinguinal approach with relatively minimally invasive limited incision in the treatment of acetabular fractures not only has good reduction effect and good recovery of hip joint function,but also has less operation blood loss,more mild postoperative pain and lower complication rate.
