16排多层螺旋CT结肠充气造影技术的临床应用研究 被引量:5

摘要 目的探讨CT仿真结肠内镜(CTVE)充气造影扫描时,比较其体位的变化及肠液,固体残渣、气体的再分布及其形态的变化,以及对结肠病变显示的敏感度评价。方法38例患者进行结肠镜(CF)检查之前,进行了结肠CTVE检查,并进行了仰卧位和俯卧位扫描根据结肠的储留液与固体残渣及各段肠管的扩张情况进行评分;比较不同体位下结肠扩张度的差异。并以结肠纤维内镜诊断为病理对照确定其对结肠疾病诊断的敏感度。结果同一体位、不同部位秩和检验,仰卧位、俯卧位P值分别<0.01,说明此4个部位得分有差别。升结肠、横结肠P值>0.05,两部位的不同体位间无差别,而降结肠、直肠与结肠P值<0.01,两部位的不同体位间有差别。CT仿真内镜技术(CT-C)对结肠病变诊断的敏感度为86.15%。对息肉大小诊断的敏感度分别为<5mm,74.60;6~10mm,81.82%,11mm以上100%,随着病变的增大其诊断的准确性明显提高。结论CTVE中进行仰卧位和俯卧位扫描对于保证结肠的充分扩张是极为必要的;对大肠疾患诊断及筛检价值的研究报告认为:5mm以下小的息肉无临床诊断价值,则对于6mm以上的肠道隆起性病变其筛查诊断、治疗有较高的临床应用价值。 Objective To compare the changes of liquor entericus,solid residues and gas redistribution and their appearances in different postures,and to evaluate its sensitivity to colon diseases when CT virtual endoscope(CTVE)is conducted.Methods 38 patients received colon CTVE before colonoscope(CF)examinaiton,and were scanned in dorsal posture and prone posture to score based on colonic retention liquid,solid residues and the expansion of intestinal canals;the differences between colonic expansibility in different ...
