同种异体血管移植的研究进展

摘要   随着血管外科的发展,手术例数的增加,血管重建也越来越重要,而血管重建就必然会涉及到血管替代材料的问题,移植物缺乏的问题逐渐成为人们关注的焦点.血管替代材料分为人工血管和生物血管,生物血管又分为自体血管、同种异体血管和异种血管.近几年来,多采用人工血管,特别是膨体聚四氟乙烯(ePTFE)的应用较为广泛,但人工血管不完全符合生理需求,其顺应性差[1],血管内皮的生成不及生物血管,静脉和小动脉(直径<3.0~4.0mm)移植的远期通畅率并不十分理想[2];另外人工血管感染、吻合口出血、假性动脉瘤、血栓形成发生率高,不仅导致手术失败导致截肢致残甚至死亡[3],而且价格昂贵.自体血管移植受到血管来源的限制,管径大小和血管的长度往往难以符合要求.异种血管由于其严重的免疫排斥反应而很少应用于临床.相比之下,同种异体血管移植有着明显优势.……
