经纤支镜高频电刀灼烧并放置支架治疗结核性左主支气管狭窄 被引量:3

摘要 目的:探讨经纤支镜高频电刀灼烧扩大管腔并放置金属支架治疗左主支气管结核性狭窄的疗效和安全性。方法:对11例结核性左主支气管狭窄并左肺不张的患者,经纤维支气管镜采用高频电刀烧灼扩大管腔并置放镍-钛记忆合金气道支架的方法治疗。结果:11例经治疗后左主支气管恢复畅通,9例全肺复张,2例下肺复张;追踪观察4个月后,2例因上、下叶以下支气管结核性肉芽肿阻塞,再发生左全肺不张。结论:用经纤支镜高频电刀灼烧扩大管腔后置放支气管金属支架的方法,治疗结核性左主支气管狭窄,比采用单一的高频电刀或置放金属支架治疗效果更好,但远期疗效及副作用有待进一步观察。 Objective: To investigated the curative effect and safety of treating left main bronchial tuberculous stenosis by means of radio knife burn through fibrous bronchoscope and placement of metal stand. Methods: 11 case of patients suffering from left main bronchial tuberculous stenosis and atelectasis of left lung were cured by both radio knife burn through fibrous bronchoscope and placing metal stand in the bronchus where tuberculous stenosis occured. Results: 11 case of patients resumed unblocked in the left main bronchus,9 recuperated in the whole lung, 2 recuperated in the lower lobe of lung. After a 4 months' tracking observation,2 cases were found to have atelectasis of left lung due to the recurrence of tuberculous granuloma obstruction in the bronchus of upper and lower lobe. Conclusion: The method of treating left main bronchial tuberculous stenosis by means of radio knife burn through fibrous bronchoscope and placement of metal stand is better than using radio knife burn through fibrous bronchoscope only or placement of bronchus stand along to cure left bronchial tuberculous stenosis.
