摘要 目的探讨强化胰岛素治疗对成年重症患者近期临床结局的影响。方法采用前瞻性、随机对照临床试验,应用随机排列表将122例随机血糖>11.1mmol/L、住ICU≥5天的重症患者分为强化胰岛素治疗组(IIT组,n=58)和传统胰岛素治疗组(CIT组,n=64),对比分析两组患者在ICU停留时间、住ICU期间感染与多器官功能障碍综合征(MODS)发生率、病死率以及ICU医疗费用的差异。结果与CIT组相比,IIT组患者ICU停留时间明显缩短[(9.01±5.28)天vs.(15.29±7.75)天,P<0.05],感染发生率(29.31%vs.43.75%,P<0.05)、MODS发生率(39.66%vs.54.69%,P<0.05)、ICU病死率(27.59%vs.45.31%,P<0.05)和ICU医疗费用[(15.04±9.77)万元vs.(28.68±10.47)万元,P<0.05]均明显降低。结论强化胰岛素治疗可以改善重症患者近期临床结局。 Objective To study the effect of intensive insulin therapy on short-term outcome in critically ill adult patients in intensive care unit(ICU).Methods Totally 122 critically ill patients with blood glucose level >11.1 mmol/L and an ICU stay length ≥5 days were randomly divided into the intensive insulin therapy group(IIT group,n=58) and the conventional insulin therapy group(CIT group,n=64).The influence of the IIT and CIT on the ICU stay length and incidence of infection and multiple organ dysfunction sy...
