摘要
目的了解阴沟肠杆菌产AmpC酶或同时产超广谱β-内酰胺酶(ESBLs)的临床现状,检测阴沟肠杆菌对常用13种抗生素的耐药性,以指导临床合理选择抗生素。方法采用三维实验确证产AmpC酶和产ESBLs菌株;用纸片扩散法进行耐药性检测。结果分离出阴沟肠杆菌120株,AmpC酶菌株检出率为20.0%(24/120),ESBLs菌株检出率为25.0%(30/120),同时产AmpC酶和ESBLs即SSBL检出率为12.5%(15/120)。阴沟肠杆菌SSBL菌株对左氧氟沙星、头孢吡肟和氨曲南的耐药率高于单产AmpC酶的菌株,差异具有统计学意义(P<0.05);SSBL菌株对亚胺培南、头孢西丁、环丙沙星的耐药率高于ESBLs菌株,差异有统计学意义(P<0.05);SSBL菌株对亚胺培南、头孢吡肟、头孢曲松、环丙沙星的耐药率高于不产AmpC酶及ESBLs即SSBL阴性的菌株,差异具有统计学意义(P<0.05)。结论AmpC酶和ESBLs是导致阴沟肠杆菌对第3代头孢菌素耐药的重要原因,SSBL菌株的流行增强了此类菌株的耐药性,治疗阴沟肠杆菌感染时应以耐药检测结果为依据,可根据药敏结果考虑选用碳青酶烯类、喹诺酮类抗生素。
