摘要
目的通过微创经皮肾镜取石术手术入路相关结构的解剖学研究,为临床手术穿刺点的选择和手术入路的解剖学层次提供形态学基础。方法对8具16侧正常成人尸体标本按临床手术体位进行穿刺,并解剖观察层次结构、毗邻关系和拍摄、测量。结果在腋后线第10肋间隙穿刺,穿破膈胸膜及膈的概率为56.25%(9/16)。在肩胛线第10肋间隙穿刺,穿破膈胸膜及膈的概率为100%。在腋后线第11肋间隙穿刺,穿破膈胸膜及膈的概率为6.25%(1/16)。在肩胛线第11肋间隙穿刺,穿破膈胸膜及膈的概率为12.5%(2/16)。在腋后线第12肋下缘肋尖处穿刺穿破膈胸膜及膈的概率为0。结论右肾在腋后线12肋肋尖下缘内侧3~4cm穿刺较安全;左肾在腋后线第12肋上、下缘肋尖内侧2~3cm穿刺较安全。
Objective To provide anatomy base of the operative route by dissecting the dependency structure of minimally invasive percutaneous nephrolit hotomy. Methods 8 corpose samples(16sides)were dissected and the correlated anatomic hierarchical structure were observed,shot and measured by simulating the operative route.
