肺纤维化的治疗现状及展望 被引量:3

摘要 肺纤维化是一种慢性的进行性肺间质疾病,肺实质逐渐被纤维瘢痕组织代替失去呼吸功能。典型的临床表现是不可逆转的呼吸困难、低氧血症,患者最终因呼吸功能衰竭死亡。大量的研究显示肺纤维化是一种预后很差的疾病,大部分患者一旦被诊断为肺纤维化,其生存时间不超过5年[1~3]。肺纤维化也是肿瘤治疗过程中的一种严重并发症,放射治疗和某些化疗药物都可引起肺纤维化。这些化疗药物(例如甲氨蝶呤、博莱霉素、环磷酰胺、长春新碱、阿霉素等)不仅可以直接损伤肺组织引起纤维化,当与放射治疗同时或先后进行还可加重放射治疗导致的肺损伤。不论是肿瘤治疗引起的肺纤维化,还是其他原因引起的肺纤维化都与炎症反应、炎性细胞、细胞因子等有密切关系[4~7]。肺纤维化的诊断要结合临床、影像学及病理等各个方面,由于其起病隐匿,早期临床表现不典型,做出早期诊断相当困难。临床上许多患者在被诊断时已是疾病的中晚期[8],这就更增加了治疗的困难。在过去的几十年中,国内外许多学者对肺纤维化的发病机制及治疗方法进行了大量的研究,但其确切的发病机制至今尚未完全明了,仍缺乏特异有效的阻止或逆转肺纤维化的治疗手段[9~11],肺纤维化的治疗仍是一项国际难题。根据目前了解的肺纤维化...
