应用同种异体骨移植治疗骨肿瘤性骨缺损 被引量:1

摘要 目的 探讨同种异体骨在治疗四肢骨肿瘤切除后大段骨缺损中的疗效,包括肢体功能恢复及并发症的发生情况.方法 2000年6月至2005年12月应用同种异体骨移植治疗四肢骨肿瘤切除后大段骨缺损患者42例,年龄14~60岁,平均38.4岁,外科治疗以Ennekimg外科分期标准为原则[1,2],对肿瘤进行切除.异体骨移植长度8~23 cm,平均12 cm.钢板固定24例,交锁髓内钉固定12例,普通髓内钉固定6例.结果 随访资料完整42例,术后随访3~62个月,平均38个月;异体骨愈合39例,不愈合3例.主要并发症:复发4例,骨不连4例,感染2例(后治愈),骨折2例,内固定失败1例.关节功能按Mankin标准评分:优17例,良13例,中8例,差4例.结论 骨肿瘤切除后的缺损需要异体骨进行重建.同种异体骨植是四肢骨肿瘤切除后大段骨缺损较为理想的修复材料.深低温保存异体骨具有保留了骨的形态、大小及强度,降低了异体骨的免疫原性,具有与自体近似的骨传导性及骨诱导性,可保留韧带附丽以供软组织重建应用等优点.异体骨移植同时是一种高并发症发生率的重建方法,降低并发症可提高肢体功能.肿瘤复发是影响治疗满意率的主要影响因素,恰当的外科边界是治疗骨肿瘤的关键.
