摘要
目的:评价糖适平对老年男性Ⅱ型糖尿病早期肾病患者的降糖疗效以及对肾功能的影响。方法:患者均为男性,年龄≥65岁,糖尿病病程在5年以上。尿微量白蛋白排泄率≥30μgmg(糖尿病肾病),Cr<177.8μmolL(肾功能不全代偿期)。其中56例使用糖适平治疗,33例使用胰岛素治疗,监测治疗前后的血糖指标和肾功能指标。结果:与胰岛素治疗比较,糖适平能有效降低老年男性Ⅱ型糖尿病早期肾病患者的空腹血糖、餐后血糖、糖化血红蛋白,但不升高尿素氮、肌苷、尿微量白蛋白排泄率,且副作用小,在本组观察中无低血糖的发生。结论:糖适平是一种安全、有效的降糖药物,可用在老年男性Ⅱ型糖尿病伴有早期肾病的患者。
Objective:To assess the curative effect of glurenorm and its influence on kidney function in elder male type Ⅱ diabetics with early diabetic nephropathy.Methods:All eligible male subjects were aged above 65 with the history of being diagnosed as diabetes for over 5 years,whose uremicroalbumin(URA)were above 30μg mg(the critiria for diabetic nephropathy)and Cr over 177.8μmol L(the limit of primer uremia).56 patients took glurenorm and the other 33 individuals were treated with insulin.Symbols reflecting plasma glucose and nephric function were compared before and after the treatment. Results:Fasting blood glucose?postprandial blood glucose and HbA1c dropped efficiently in elder male patients of glurenorm group compared with insulin group,and no difference was observed in urea nitrogen(UN)?Cr and URA between the two groups.No incidence of hypoglycemia was detected.Conclusion:Glurenorm is a kind of safe and effective regent,which can be used to elder male diabetics with primer nephropathy.
