血浆同型半胱氨酸与脑梗死的临床观察

摘要 目的观察脑梗死病人血浆同型半胱氨酸与脑梗死的关系及其临床意义。方法 79例急性脑梗死患者和同期39例对照者采用荧光偏振免疫分析法测定空腹血浆同型半胱氨酸,采用化学发光法测定叶酸和维生素 B_(12)。结果血浆同型半胱氨酸(Hcy)>15μmol/L 者,脑梗死组为57例,占71.25%。对照组为5例,占12.5%。两组平均 t-Hcy 分别为22.40±11.23 μmol/L和11.89±2.50 μmol/L,差异有极显著性(X^2=36.87,P<0.01)。叶酸水平:两组分别为4.73±1.83 ng/mL 与(6.77±4.29)ng/mL,(t=3.62,P<0.01);维生素 B_(12)水平:两组分别为5.59±0.73 pg/mL 与5.96±0.56 pg/mL,(t=2.78,P<0.01);差异均有极显著性。结论脑梗死病人血浆同型半胱氨酸水平是升高的,而叶酸和维生素 B_(12)水平呈下降。 Objective To investigate the relationship between plasma homocysteine and acute cerebral infarction and clinical meaning.Methods Fasting plasma homocysteine was determined by fluorescence polarization immuno- assay in 79 patients with acute cerebral infarction and 39 controls.Plasma folie acid and vitamin B_(12)were determined by automatic chemiluminescence system.Results There were 57 cases with increased plasma homocysteine level (>15μmol/L)in the actue cerebral infarction group and 5 cases in the control group,accounting for 71.25% and 12.5% respectively.Mean plasma homocysteine level was 22.40±11.23μmol/L in the actue cerebral infarction group and 11.89±2.50μmol/L in the control group with significant diffrence(x^2=36.87,P<0.01).Plasma fo- lic acid level was 4.73±1.83 ng/mL and 6.77±4.29 ng/mL,(t=3.62,P<0.01).Plasma vitamin B_(12)level was 5.59±0.73 pg/mL and 5.96±0.56 pg/mL,(t=2.78,P<0.01).Conclusion Plasma level of homocysteine is higher in the patients with acute cerebral infarction.However,plasma level of folic acid and vitamin B_(12)is lower.
