摘要
目的应用实时三维超声心动图探讨左心病变相关的肺高压(pulmonary hypertension,PH)患者右心室形态和收缩功能的特征。方法入选左心病变相关的PH患者23例和健康对照者27例,行常规超声心动图检查,采集右心室实时三维超声心动图图像并进行定量分析,获得右心室整体和流入道部、体部、流出道部三个局部的容积及收缩功能的参数指标,与正常对照组进行比较,并进一步分析三维超声参数与常规超声心动图参数的相关性。结果 PH组右心室整体、流入道部、体部、流出道部的舒张末容积(EDV)、收缩末容积(ESV)均显著大于对照组(均P<0.001);右心室整体、流入道部、体部射血分数(EF)均显著小于对照组(均P<0.01),而流出道部EF值与对照组之间差异无统计学意义(P>0.05);右心室流入道部和体部每博输出量(SV)与对照组差异无统计学意义(P>0.05),而整体与流出道部SV略高于正常对照组(P<0.05)。PH组右心室整体ESV和流入道部ESV与肺血管阻力之间呈显著正相关(r=0.69,P<0.001;r=0.62,P<0.001),右心室整体EF和流入道部EF与肺血管阻力之间呈显著负相关(r=-0.72,P<0.001;r=-0.67,P<0.001)。结论左心病变相关的PH患者右心室容积增大,右心室整体和局部收缩功能减退,右心室形态和收缩功能受损的程度与肺血管阻力密切相关。
Objective To assess right ventricular volume and function in patients with pulmonary hypertension secondary to left-sided heart disease using real-time three-dimensional echocardiography. Methods Two-dimensional and real-time three-dimensional echocardiography ( RT-3D) was performed in 23 patients with PH secondary to left-sided heart disease and 27 normal controls. All RT-3D images were analyzed quantitatively to generate global and regional ( inflow,body and outflow part) volumetric and functional data of the right ventricle,which was compared between the two groups and further correlative analysis was carried out. Results Global and regional end-diastolic and end-systolic volumes ( EDV,ESV) of the right ventricle in the PH group were significantly larger than that in the control group ( P 0. 05 for all). Global and inflow ESV of the right ventricle correlated positively with pulmonary vascular resistance( r = 0. 69,P < 0. 001; r = 0. 62,P < 0. 001) ; Global and inflow EF of the right ventricle correlated negatively with pulmonary vascular resistance( r = -0. 72,P < 0. 001; r = -0. 67,P < 0. 001). Conclusions Right ventricular global and regional volume was enlarged and systolic function was impaired in patients with pulmonary hypertension secondary to left-sided heart disease,which was changed in liner relationship with pulmonary vascular resistance.
