乌司他丁对术中单肺通气患者术后肺内分流及动脉血氧合的影响

摘要 目的观察术中单肺通气患者术中和术后24小时的动脉血氧合以及肺内分流的变化,并研究乌司他丁对单肺通气患者术后保护作用。方法对术中需行单肺通气的肺叶切除术患者60例ASAⅡ~Ⅲ级,随机分为对照组P以及术前使用乌司他丁5000U/kg的实验组W。每组均为30例,手术开始时,手术结束时,术后24h分别进行动脉血及混合静脉血作血气分析,并计算Qs/Qt及PaO2。结果手术结束时,两组Qs/Qt均有增加(P<0.05),然而实验组术后24h在Qs/Qt是低于对照组(P<0.05)。手术结束时两组动脉血氧合与术前无明显差异。在术后24小时,两组的PaO2/FiO2是低于单肺通气前,但术后24小时,实验组在PaO2/FiO2高于对照组(P<0.05)。结论乌司他丁能够有效改善开胸手术术中单肺通气患者术后动脉血氧合状况,减少术后肺内分流。 Objective To obserate the unilateral palmonary ventilation during operation and Change of arterial oxygenation and intrapulmonary shunt after operation 24 hours.And the ulinastatin for patients with unilateral pulmomary ventilatoom in the protective effects of postoperation were studied.Methods 60 cases ASA Ⅱ~Ⅲgrade with unilateral plmonary ventilation were randomly divied into control group P and experimental group W(using ulinastatin).The arterial blood and mixed venous blood samples were taken respectively during operative beganing,operative ending and after operation 24 hours.The Qs/Qt and PaO2/FiO2 were calculated by formula. Results In the two groups,the Qs/Qt was increased at the end of operation(P< 0 05). However,the group W was lower than that group P in the Qs/Qt after operation 24 hours(P< 0 05). The arterial oxygenation of two groups hadn′t obvious difference during the end of operation and before operation.Two groups PaO2/FiO2 were lower than that before unilateral pulmonay ventilation and after operation 24 hours(P< 0 05),but the group W was higher than that group P in the PaO2/FiO2 after operation 24 hours. Conclusion The ulinastatin can effactively improve postoperative artericl oxygenation for patients with thoracic surgery of intraoperative unilateral pulmonary ventitation and reduce postoperative intrapulmonary shunt.
