摘要
目的:探讨女性代谢综合征与血清促肾上腺皮质激素(ACTH)及皮质醇(COR)水平的关系。方法:以标准法测量30名女性代谢综合征患者的血压、腰围(WC)、体重指数(BMI)、腰臀比(WHR),并测定其血清ACTH、COR、空腹胰岛素(FINS)、空腹血糖(FBG)、血脂水平,另取30名健康志愿者进行比较。结果:⑴女性代谢综合征组血清ACTH和COR水平均显著高于对照组(P<0.01);⑵在控制了年龄、腰围、腰臀比后,COR水平与HOMA-IR、血糖水平呈显著正相关(r=0.46,r=0.41,P<0.01);⑶多因素分析提示ACTH、COR、FINS为代谢综合征发病的危险因素。结论:血清ACTH和COR水平升高是女性代谢综合征发病的危险因素,ACTH和COR可作为临床早期诊断代谢综合征的敏感指标。
Objective To explore the association between the serum levels of adrenocorticotrophic hormone(ACTH),cortisol(COR)and metabolic syndrome(MS)in female.Methods The blood pressure,waist circumference(WC),body mass index(BMI),waist-to-hipratio ratio(WHR)were measured with standard method in 30 women with metabolism syndrome,meanwhile,the serum levels of ACTH,COR,fasting insulin(FINS),fasting blood glucose(FBG)and TG were determined.Another 30 healthy volunteer were selected as control group.Results The serum levels of ACTH and COR in MS group were all very significantly higher than those of control group(P<0.01);⑵After excluding the influence of age,WC and WHR,the serum level of COR were all positively correlated with HOMA-IR and FBG(r=+0.46,r=+0.41,P<0.01);⑶Multiple regression analysis indicated that ACTH,COR,FINS were independent relative factors of metabolic syndrome in female.Conclusion The increasing levels of serum ACTH and COR were risk factors of metabolic syndrome,and the two indexes were sensitive for clinical diagnosis in early metabolic syndrome.
