弹性成像定量分析评价肝脏硬度的初步研究 被引量:11

Evaluation of hepatic stiffness by ultrasonic elastography quantitative analysis: preliminary experience

摘要 目的探讨基于弹性成像技术的组织弥散定量分析软件评估肝脏硬度的可行性。方法应用组织弥散定量分析软件分析30名体检健康者(健康对照组)与30例肝硬化患者(肝硬化组)的肝脏硬度,得到11个弹性图像的特征量,包括应变均值、标准差、蓝色区域%、复杂度、峰度、偏度、对比度、均等性、复杂度、一致性、相关性,分析两组间该11个特征量的差异。结果健康对照组肝脏弹性图像特征量的应变均值、标准差、蓝色区域%、复杂度、峰度、偏度、对比度、均等性、复杂度、一致性、相关性分别为109.836±3.637、56.360±5.956、20.235±4.432、34.600±10.806、2.506±0.182、0.277±0.116、243.283±68.728、3.931±0.061、0.108±0.015、0.000±0.000、0.961±0.008,肝硬化组的分别为79.843±12.082、73.830±4.314、49.932±9.529、54.349±15.169、2.634±0.447、0.776±0.261、329.266±71.331、3.547±0.241、0.219±0.058、0.019±0.014、0.969±0.009。两组相比较,11个弹性图像的特征量中仅峰度的差异无统计学意义,其余特征量的差异均有统计学意义(t=-13.019、13.011、15.467、5.807、9.561、4.754、-8.453、9.990、7.577、3.482,P均<0.05)。对11个弹性图像的特征量行Logistic回归分析,得出蓝色区域%是最重要的特征量。结论基于弹性成像技术的组织弥散定量分析软件有助于肝脏硬度的评估,蓝色区域%是最重要的特征量。 Objective To explore the feasibility of evaluation of hepatic stiffness by ultrasonic elastography using tissue dispersion quantitative analysis software. Methods Tissue dispersion quantitative analysis software was applied to 30 health control people and 30 liver cirrhosis patients to analyse the hepatic stiffness. Eleven characteristic quantities of elastography imaging were obtained by the tissue dispersion quantitative analysis software, including average relative strain value (MEAN), standard deviation of relative strain value (SD), area ratio of low-strain region (%AREA), complexity (COMP), kurtosis (KURT), skewness (SKEW), contrast (CONT) , entropy (ENT), inverse different moment (IDM), angular second moment (ASM), and correlation (CORR). The differences of 11 characteristic quantities between two groups were analyzed. Results The MEAN, SD, %AREA, COMP, KURT, SKEW, CONT, ENT, IDM, ASM, CORR of elastography imaging obtained by the tissue dispersion quantitative analysis software in health control people were 109.836±3.637, 56.360±5.956, 20.235±4.432, 34.600±10.806, 2.506±0.182, 0.277±0.116, 243.283±68.728, 3.931±0.061, 0.108±0.015, 0.000±0.000, 0.961±0.008, and in liver cirrhosis patients were 79.843±12.082, 73.830±4.314, 49.932±9.529, 54.349±15.169, 2.634±0.447, 0.776±0.261, 329.266±71.331, 3.547±0.241, 0.219±0.058, 0.019±0.014, 0.969±0.009, respectively. The characteristic quantities had statistically significant differences between two groups except KURT ( t=-13.019,13.011,15.467,5.807,9.561,4.754,-8.453,9.990,7.577,3.482,P<0.05). The Logistic regression analysis was applied to the 11 characteristic quantities and it showed that the %AREA was the most important one. Conclusions The tissue dispersion quantitative analysis software is helpful in the evaluation of hepatic stiffness. The %AREA is the most important characteristic quantity.
