摘要 目的:探讨骨梗死的影像表现特点。材料与方法:回顾性分析经病理或临床证实的28例骨梗死的X线、CT和MRI的影像表现。结果:多发21例,单发7例,累及61个骨骼。X线检查8例,表现为散在的斑点状、斑片状、条纹状或呈蜿蜒状硬化的密度增高影;CT检查7例,CT表现为斑片状、条纹状、环状、磨玻璃样密度;MRI检查17例,其中急性期2例,表现为骨质内多发小斑片状的异常信号,呈长T1W1、长T2W1,脂肪抑制序列上呈高信号,边界尚清。亚急性期5例,表现梗死区中间环T1WI、T2WI加权像上均呈低信号,外围呈现长T1W1、长T2W1信号,脂肪抑制序列上呈等信号,边缘伴有线样低信号环;慢性期10例,在T1WI、T2WI上表现为梗死区中央和边缘均呈低信号,形态不规则,呈地图样。结论:X线和CT对早期骨梗死诊断价值有限,MRI对骨梗死各期表现具有很高的敏感性和特异性,三者结合有助于诊断。 Purpose:To investigate of the characteristic findings images of bone infarction.Materials and Methods:X-ray and CT and MRI images were performed of clinical and pathology in 28 cases with bone infarctions were analyzed retrospectively.Results:21 cases are sporadic,Single 7 cases,involving the 61 bone.X-ray examination of 8 cases,the scattered speckle,patchy,stripe or a winding of atherosclerosis density increases shadow.CT examination of 7 cases,CT is patchy,stripe,ring,ground glass kind density change.MRI examination of 17 cases,the acute phase of 2 cases,the multiple small patchy abnormal signals,a long T1W1and long T2W1 change,the high signal of fat suppression,clear border;the.subacute phase of 5 cases,all low signal of T1WI and T2WI in infarction area intermediate riag,long T1W1 and long T2WI in the peripheral,the etc signal of fat suppression,low signal riag of line-like in edge.The chronic phase of 10 cases,all low signal of T1WI and T2WI in central and edge of in infarction area,irregular shape,earth drawing were showed.Conclusion:X-ray and CT for early diagnosis of limited value,MRI detected in each period of bone infarction performance with high sensitivity and specificity,the combined determination the X-ray and CT and MRI in bone infarct is helpful to diagnosis.
