摘要
目的双源双能量CT是近年来发展起来的新的成像技术,对于血管成像具有独到的优势。迄今国内尚未见有该方法在富血供肝肿瘤方面的应用研究。文中初步探讨动脉期双能量CT对富血供肝肿瘤的临床应用价值。方法对30患者行上腹部动脉期双能量CT扫描,同时获得140 kVp、80 kVp及两者按0.3比例融合的加权图像。分别测量3种图像的病变部位、邻近正常肝组织及腹部前壁均匀脂肪的密度,以病变肝组织与邻近正常肝组织的密度差表示病灶清晰度。计算3种图像的对比噪声比(contrast-to-noise ratio,CNR)。采用3分方法评价3种图像的图像质量。采用随机区组的秩和检验进行统计学分析,组间两两比较采用LSD检验。结果检出23例不同肝病患者,共34个病灶。3组病灶清晰度差异有统计学意义(P=0.001)。80 kVp的病灶清晰度[(33.15±15.6)HU]明显高于加权图像[(21.0±10.7)HU],P=0.011和140 kVp图像[(17.8±8.4)HU],P=0.002。3组对比噪声比差异有统计学意义(P=0.001),80 kVp的CNR(2.49±1.15)高于140 kVp(1.5±0.9),P=0.04及加权图像(1.96±0.8),P=0.012),而140 kVp图像与加权图像比较,病灶清晰度(P=0.517)及CNR(P=0.145)均无无统计学意义差异。加权图像质量评分最高(2.5±1.2),80 kVp图像次之(2.2±0.9),140 kVp最低(2.0±0.8),3种图像质量总体评分差异无统计学意义(P=0.12)。结论动脉期双能量CT能够提高富血供肝肿瘤的清晰度,图像质量能够满足临床诊断要求,具有较好的临床应用前景。
Objective Dual-dource dual-energy CT,as a new imaging technique,has its unique advantages in blood vessel imaging.The purpose of this study is to determine the clinical value of dual-source dual-energy CT for patients with hypervascular liver tumors during the hepatic arterial phase.Methods Thirty patients with suspected liver disease underwent contrast-enhanced dual-source dual-energy CT,and three data sets were obtained: 140 kVp,80 kVp and a fused image with an 80:140 kVp linear weighting of 0.3.A region-of-interest cursor(ROI) was placed within the tumor and the adjacent liver.The attenuation differences between the tumor and the adjacent liver and the contrast-to-noise ratio(CNR) were calculated for hypervascular liver tumors at 80 kVp,weighted average 120 kVp and 140 kVp.The quality of the images at 80 kVp and weighted average were evaluated using a 3-point scale.Randomized block rank test was used to make paired comparisons.Results A total of 34 lesions were detected in 23 liver patients.The mean differences in CT attenuation between hypervascular liver tumors and adjacent liver were significantly higher at 80 kVp(33.15±15.6) than at weighted-average 120 kVp(21.0±10.7)(P=0.001) and 140 kVp(17.8±8.4)(P=0.001),and so was CNR at 80 kVp(2.49±1.15) than at weighted-average 120 kVp(1.96±0.8)(P=0.012) and 140 kVp(1.50±0.9)(P=0.04).The image quality at 80 kVp(2.2±0.9) was similar to that at weighted average 120 kVp(2.5±1.2) and that at 140 kVp(2.0±0.8)(P=0.12).Conclusion Contrast-enhanced dual-source dual-energy CT improves the conspicuity of hypervascular liver tumors during the late hepatic arterial phase,and the images can meet the requirements of clinical diagnosis.
