摘要
目的检测妊娠早期产妇血清甲状腺激素的水平并加以探讨,评估其临床参考值范围,对比育龄期女性中妊娠早期人群和非妊娠人群间血清甲状腺激素水平的差异。方法收集了315例妊娠早期(14~20周)健康孕妇血清和83例非妊娠健康育龄妇女的血清,用化学发光法测试血清甲状腺激素系列指标的浓度水平,包括血清总三碘甲腺原氨酸(TT3)、血清总甲状腺素(TT4)、血清游离三碘甲腺原氨酸(FT3)、血清游离甲状腺素(FT4)、血清促甲状腺激素(TSH)水平并对结果分别进行了统计学描述,初步建立了实验室参考值范围;使用SPSS19.0软件对妊娠组和非妊娠组标本的结果进行了多元方差分析,评估了两组间差异的显著性。结果妊娠早期妇女血清甲状腺激素参考范围为FT33.64~5.98pmol/L、FT411.26~18.40pmol/L、TT31.06~1.92ng/ml、TT47.16~14.38g/dl、TSH 0.2~3.59μIU/ml;育龄女性妊娠组和非妊娠组的甲状腺激素结果具有显著性差异,TT3、TT4、FT3妊娠组测试结果高于非妊娠组,FT4、TSH妊娠组测试结果低于非妊娠组。结论妊娠早期血清甲状腺功能检测采用多指标联合检测可优势互补。TSH作为筛查指标,FT4可作为反应疗效的敏感指标。
Objective To investigate the level of serum thyroid hormone panel in early normal pregnant women ,make a preliminary estimation of the normal range;investigate the significance of serum thyroid hormone between the pregnant team and non-pregnant team.Method Serum concentration level of thy-roid hormone panel,including TT3 ,TT4 ,FT3 ,FT4 and TSH,in 315 early pregnant subjects and 83 cases of non-pregnant fertile women were detected with chemiluminescence on technical platform .The result is statistical description and the in-house normal range is set up.The significance between the pregnancy and non-pregnancy team was also evaluation by multiple abstract variance analysis with SPSS 19.0 software. Results Reference range in early-stage pregnant women is:FT3 3.64-5.98 pmol/L,FT4 11.26-18.40 pmol/L,TT3 1.06-1.92 ng/ml,TT4 7.16-14.38 g/dl,TSH 0.2-3.59μIU/ml.Serum con-centration of thyroid hormone panel shows significantly different between pregnancy and non-pregnancy women team.For TT3 ,TT4 ,FT3 ,the pregnant women team shows higher serum concentration than the that of the non-pregnancy team,while for FT4 and TSH,The result is the opposite.Conclusions By in-vestigating several items about the level of serum thyroid hormone panel in early normal pregnant women they can complement each other’s advantages.TSH can be used in the screening process while FT4 can be used as the sensitive referent of clinical curative effect.
