正常高值血压与肾功能的相关性研究 被引量:3

摘要 目的高血压常并发靶器官损害。本研究的目的是观察正常高值血压患者肾功能的变化,以探讨正常高值血压患者是否存在与血压升高相关的靶器官损害,以指导其在高血压治疗和预防方面的意义。方法根据JNC-7诊断标准筛选病例并分为三组:正常血压组(NT组)为56例正常血压健康人,男性41例,女性15例,平均年龄(45.54±6.1)岁;正常高值血压组(PH组)为51例正常高值血压患者,男性37例,女性14例,平均年龄(46.04±6.2)岁;高血压组(EH组)为54例原发性高血压患者,男性40例,女性14例,平均年龄(49.06±5.3)岁;留取晨尿,抽取静脉血,离心后采用放免法和酶法测定尿β2-微球蛋白(β2-MG)、尿微量白蛋白(mAIb)及血尿素氮(BUN)、血肌酐(CR)。结果与NT组相比,PH组和EH组尿β2-微球蛋白(β2-MG)、尿微量白蛋白(mAIb)水平明显升高(P<0.05;P<0.0 1)。血尿素氮(BUN)、血肌酐(CR)在EH组明显升高,与NT组比较P<0.01;在PH组血尿素氮(BUN)轻度升高,与NT组比较无显著性差异;血肌酐(CR)明显升高,与NT组比较P<0.01。结论 PH组患者尿β2-MG、尿微量白蛋白(mAlb)、血肌酐(CR)出现了异常变化,提示正常高值血压患者可能存在与血压升高相关的肾损害。 Objective This study investigates the renal function in prehypertensive patients.Methods Patients were scruited according to JNC-7 and divided into three groups:56 normotensive subjects(NT group,41 males,mean age 45.54±6.1 years old);51 prehypertensive patients(PH group,37 males,mean age46.04±6.2 years old);and 54 hypertensive patients(EH group,40 males,mean age49.06±5.3 years old).Serum lipids and urine B2-MG,urine mAIb,serum BUN and CR by radioimmunoassay and enzyme linked immnosorbent assay.Results urine B2-MG and urine mAIb were significantly higher in the PH and EH groups compared to NT group(P<0.05;P<0.01).Serum BUN and CR were significantly increased in the EH group than that in NT group(P<0.01).Serum BUN was little increased in the PH group,it wasn't significance of difference PH group and NT group;But serum CR was significantly increased in the PH group than that in NT group(P<0.01).Conclusin Urineβ2-MG、urine mAIb and serum CR were increased in prehypertensive patients.
