摘要
目的探讨重组人促红细胞生成素(rhEPO)对心肺复苏后脑损伤的保护作用。方法雄性大鼠75只,随机分为3组,对照组(C组),肾上腺素组(AD组),肾上腺素+rhEPO组(E组),每组25只。麻醉后气管切开行机械通气。夹闭气管导管法建立大鼠心跳呼吸停止模型。E组大鼠在窒息前24h、16h腹腔注射rhEPO3000U/kg,窒息前10min静脉注入rhEPO30U/kg。AD组和E组呼吸心跳停止7min后恢复通气,经主动脉注入复苏合剂进行复苏。C组不予以窒息、复苏。C组动脉置管15min后,AD组、E组在自主循环恢复后1h、2h、3h采集脑组织标本及动、静脉血,进行血气分析。结果两组初期复苏率无显著差异(P>0.05)。与AD组比较,E组各时点颈动-静脉氧含量差(Ca-jO2)及脑氧摄取率(CEO2)明显升高(P<0.05)。同时电镜结果显示,E组超微结构改变明显减轻。结论促红细胞生成素虽不能提高初期复苏率,但能够减轻继发性脑损伤,增加脑氧摄取率,改善脑氧合功能。
Objective To evaluate the effects of recombinant human erythropoietion (rhEPO)given before cardiopulmonary resuscitation (CPR)on postresuscitation cerebral injury. Methods Seventy-five male Wistar rats (200~300 g)were randomly divided into 3 groups: group I control(C,n =25);groupⅡepinephrine(AD,n =25)and group Ⅲepinephrine+erythropoietin(E,n =25). After anesthesia,all animals were taken mechanical ventilation by incision of trachea and made model of cardiorespiratory arrest. Before asphyxia for 24 and 16 hours,erythropoietin(EPO 3 000 U/Kg)was injected into abdominal cavity,and EPO(30 U/kg)was injected into vein at 10 min before asphyxia. CPR was started after 7 min of cardiorespiratory arrest and epinephine 100 μg/kg was given via carotid artery. Group C was subjected to no asphyxia or resuscitation .Arterial and jugular venous blood and brain tissue sample were collected on the point 1 h,2 h,and 3 h after ROSC in group AD and E. The corresponding data from C group was assigned to 15 min after surgical preparation. Results There is not obvious different basic resuscitation rate in the two groups. CEO2and C(a-jv)O2were significantly increased as compared to the value in group AD (P < 0.05). Electron microscopy showed that ultrastructural changes in Group E significantly were reduced. Conclusion Although rhEPO can not improve the basic resuscitation rate,it can relieve the injury of cerebral ultrastructure,increase cerebral extraction rate of oxygen,and improve cerebral oxygenation in post-resuscitation rats.
