摘要
目的 探讨乳腺癌MRI形态学表现与病理、分子生物学之间的相关性。方法 对 78例原发性乳腺癌患者术前行乳腺MR扫描 ,分析乳腺癌的MRI形态学表现。术后标本行免疫组织化学染色测定癌细胞雌激素受体 (ER)、孕激素受体 (PR)、C erbB 2和P5 3的表达情况及微血管密度 (MVD)分布情况 ,并分析与MRI表现之间的关系。结果 80个乳腺癌中 ,癌肿毛刺状边缘与癌细胞ER阳性表达呈正相关 (P <0 0 1) ,肿瘤T分期与癌细胞ER阳性表达呈负相关 (P <0 0 5 ) ,肿瘤内部坏死与癌细胞C erbB 2、P5 3阳性表达呈正相关 (P <0 0 5 ) ,而肿瘤毛刺状边缘、边界模糊、内部坏死、T分期与癌细胞PR表达水平无显著相关性 (P >0 0 5 )。 4 1个乳腺癌中 ,癌肿动态增强MR对比剂空间分布与相应MVD之间存在高度正相关 (P <0 0 1)。结论 乳腺癌MRI形态学表现与病理、分子生物学之间存在一定相关性 ,可根据乳腺癌MRI表现对癌灶的生物学行为和预后进行评估。
Objective To investigate the correlation among MRI morphologic features, pathology, and molecular biology of breast cancer.Methods MR scanning was performed in 78 patients with breast cancer before operation and MRI morphologic features of breast cancer were analyzed.The mastectomy specimens of the breast neoplasm were stained with immunohistochemistry, and the expression of estrogen receptor (ER), progesterone receptor (PR), C-erbB-2, p53, and the distribution of microvessel density (MVD) was measured.The pathologic results were compared with MRI features.Results Among the 80 breast cancers,ER positive expression was positively correlated with the spiculate margin of breast cancer (P<0.01).ER positive expression showed a significant inverse correlation with T-stage (P<0.05).C-erbB-2 and p53 positive expression were positively correlated with the necrotic center of breast cancer (P<0.05).And the expression of PR was not significantly correlated with spiculate margin, obscure margin, necrotic center, and poorly-defined T-stage of breast cancer (P>0.05).Among the 41 breast cancers with dynamic MR scans, there was positive correlation between the spatial distribution of contrast agent and MVD (P<0.01).Conclusion There exists some correlation among MRI morphologic features, pathology, and molecular biology factors in breast cancer to certain extent.The biologic behavior and prognosis of the breast cancer can be assessed according to MRI features.
