TIPSS对门脉高压患者血浆液递物质及门脉系统血流动力学的影响

摘要 目的 经颈内静脉肝内门静脉内支架分流术。方法 运用彩色超声多普勒进行门脉系统血流动力学检测。结果 术后一周,门静脉、肠系膜上静脉、脾静脉血流速度分别加快达128%、65%及73%、血流量显著增加(P<0.01)肝外门体分流量由术前的1093ml/min降至264ml/min。TIPSS前后门脉血流量与胰高糖素与血管紧张素Ⅱ水平之比呈显著正相关。结论 TIPSS术后门脉系统血流动力学变化导致液递物质水平改变。扩血管物质与缩血管物质比例增大。 Objective Transjugular intrahepatic portosystemic sfent shunt(TIPSS). Methods Measuring portal and heptic vein pressure before and after TIPSS creation. Results A week after shunting, the mean of the Maxium blood flow velocity in the main portal vein, supeciormensentang vein and spleen vein increased by 128%, 65%, and 73% respectively, blood flow was creased significantly (P<0. 01) .The blood flow of extraheptic portosystemic shunt decreased from 10931/ min to 64ml/min. Signcficant positive correlation was found between glucagon/angiotensin I level and portal vein blood flow before and after shunting. Conclusion It was suggested that after TIPSS creation portal system hemodynamics change caused change of plasma humoral substance level, vasodilator/vasocon tractor ratio increased.
