创伤和静脉营养支持对血清脂类代谢的影响

摘要 本文通过测定胃癌根治术前后的血清总胆固醇、甘油三酯、三种脂蛋白及二种载脂蛋白的变化,以了解腹部大手术后应用静脉营养支持(IVN)对脂质代谢的影响。结果表明,胃癌根治术后脂质和脂蛋白有其特定的代谢规律,应用脂肪乳剂不增加体内脂质的负荷,IVN 支持不会扰乱术后脂质和脂蛋白的代谢模式。 Lipid and lipoprotein changes have been investigated in 28 patients with gastric cancer patients for radical gastrectomy.The results sho- wed that mean total serum cholesterols reduced during the postoperative peri- ods.2 days after operation,serum high-density lipoprotein (HDL-CH) obvi- ously increased,and significantly decreased on the 5th and 8th days (P<0.05). There were no abnomalities in serum low-density lipoprotein (LDL-CH) after operation.Serum very low-density lipoprotein (VLDL-CH) and triglycerides (TG) declined,2 days after operation and then reached the preoperative level on 5th and 8th days after operation.Furthermore,we found that changed lipoprotein levels were associated with changed levels of apolipoprotein and that there were specific changes in the levels of different apoproteins.The changes of sreum lipids and lipoproteins were similar in IVN group to the control group in addition to serum VLDL-CH and TG levels not decreasing on postopera- tion day 2. This study demonstrated that there existed a time course for the changes of serum lipoproteins and apolipoprotein A and B after major surgical trauma in man.The specific change patterns after major operation were not signifi- cantly disturbed by recieving short-term parenteral nutritional support.
