摘要
目的:观察治疗前鼻咽癌患者血清胸甘激酶1(TK-1)水平与临床特征之间的关系。方法:77例鼻咽癌初治患者(治疗组)治疗前采用酶增强化学发光技术检测TK-1浓度,选取20名健康志愿者为对照组。收集所有患者临床特征资料,并予首程根治性放疗,随访6个月,进行近期疗效评价;所有资料经SPSS 13.0软件统计包处理,进行t检验、检验和单因素方差分析(ANOVA)。结果:治疗组治疗前TK-1浓度(3.60±1.78)ρmol/L高于对照组(0.49±0.37)ρmol/L,t=-7.737,P=0.000;鼻咽癌患者临床特征中颈动脉鞘受累、颅底受侵、颅神经受累、咽后淋巴结肿大、茎突前间隙受侵及VCA-IgA与TK-1浓度有相关性,P值均<0.05;T分期、N分期、临床分期及近期疗效与治疗前TK-1浓度显著相关,P值均<0.05;而TK-1阳性率与茎突后间隙、N分期、临床分期和淋巴结转移部位有相关性,P<0.05。其他临床特征与治疗前TK-1浓度无相关性。结论:鼻咽癌患者在治疗前检测血清TK-1浓度有重要的临床意义;TK-1浓度越高,则临床分期越晚,预后相对较差。
OBJECTIVE:To observe the relationship of TK-1 levels and clinical characteristics of NPC before treatment.METHODS:Enzyme enhanced chemiluminescence was used to detect TK-1 concentration of 77 cases of NPC patients and 20 healthy people considered as the control group.All patients had received radical radiotherapy in the first process and followed up for six months and evaluated the theraputic effect.The methods that were used to analyze the data were t and chi-squre test and single factor analysis of variance(ANOVA).RESULTS: TK-1 levels of NPC group was higher than that in control group(3.60±1.78 ρmol/L vs 0.49±0.37 ρmol/L,t=-7.737,P=0.000);clinical characteristics of NPC,including involvement of carotid sheath,skull base and cranial nerve,retropharyngeal lymph nodes,spare before styloid and VCA-IgA,and TK-1 levels were related(P<0.05);T stage,N stage,clinical stage,the recent efficacy and TK-1 levels was significantly related(P<0.05).Post styloid spact,N staging,clinical stage,lymph node metastasis and the positive rate of TK-1 was related(P<0.05).Other clinical features were not related with TK-1 concentration before treatment.CONCLUSION:The detection of serum TK-1 concentration in NPC patients before treatment has important clinical significance;the high concentration of TK-1 usually indicate the later clinical stage and poor prognosis.
