摘要
目的观察Meek微型皮片移植术(以下简称Meek植皮术)治疗Ⅲ度烧伤超过80%TBSA的临床疗效。方法回顾分析2005年5月至2012年12月采用Meek植皮术治疗Ⅲ度烧伤超过80%体表总面积(total body surface area,TBSA)16例的临床资料(Meek组),并与2001年1月至2006年12月采用自体微粒皮移植+大张异体皮覆盖术治疗的Ⅲ度烧伤超过80%TBSA 16例(微粒皮组)进行比较。两组患者在性别、年龄、烧伤原因、烧伤面积、烧伤后至入院时间等一般资料比较,差异无统计学意义(P>0.05),具有可比性。同时,对两组患者的皮片移植成活率、植皮区皮片融合时间、手术次数、全身创面愈合时间、每1%烧伤面积治疗费用等进行比较并作统计学分析。结果 Meek组术后皮片移植成活率[(98.03±2.29)%]、植皮区皮片融合时间[(14.32±3.13)d]、手术次数(5.1次)、全身创面愈合时间[(56.45±5.62)d]、每1%烧伤面积治疗费用[(7131.67±789.42)元],两组指标比较,差异有统计学意义(t=10.1541、6.7007、7.4561、11.895、6.8358,P<0.05)。术后随访瘢痕增生及大关节功能情况,均明显优于微粒皮组。结论 Meek植皮法是治疗Ⅲ度烧伤超过80%TBSA的有效方法之一。
Objective To evaluate the efficiency of the micro Meek skin grafting treatment on third- degree burns with burned area exceeding 80% of the total body surface area( TBSA). Methods A retrospective analysis included 16 patients with Meek skin grafting treatment for third-degree burns over 80% TBSA from May 2005 to December 2012( Meek group),and 16 patients with autologous skin graft particles plus large covering sheet for third-degree burns over 80% TBSA from January 2001 to December 2006( microparticle group). The general information of the patients,including gender,age,cause of burn,burn area and general admission time were compared between two groups,and no statistically significant difference was found( P > 0. 05). Also,skin graft survival,graft integration time,total number of operations,general wound healing time,treatment costs per 1% burn were compared between two arms. Results The results of skin graft survival,graft integration time,total number of operations,general wound healing time,treatment costs per 1% burn obtained from patients in Meek group were much better than those in microparticle group( t =10. 1541、6. 7007、7. 4561、11. 895、6. 8358,P < 0. 05). Conclusion Meek skin grafting is very effective in treating patients with > 80% TBSA third-degree burns.
