微螺种植支抗钉即刻加载不同力值下愈合界面的比较研究 被引量:1

摘要 目的:将国产自攻型微螺种植支抗钉植于实验动物后即刻加载不同力值,12周后处死实验动物,研究其骨界面的结合及骨组织改建情况,评价自攻型微螺种植支抗钉的即刻负载能力和稳定性,为其临床应用进一步提供实验依据。方法将60枚直径1.6 mm、长度9 mm的微螺种植支抗钉局麻下植入30只兔的下肢胫骨,每只兔子2枚,两枚钉之间交互牵引。30只兔子随机分为3组,分别加力0 g(对照组)、100 g、300 g。12周后处死实验兔,采集切片图片,观察骨组织-钉愈合界面并进行静态骨组织测量。结果(1)加载不同力值的微螺种植钉中,150 g组骨结合率高于对照组和300 g组,300 g组骨结合率小于0 g组,且差异均有统计学意义(P<0.01)。(2)加力组的压力侧结合率略高于张力侧,但150 g组的压力侧与张力侧差异无统计学意义(P>0.05),300 g组压力侧与张力侧两者比较差异有统计学意义(P<0.05)。结论(1)骨结合率均达到70%~80%,即刻加载0~300 g的矫治力没有影响微螺钉种植体的稳定性。(2)力值过大对种植钉的骨结合有一定的影响。(3)对照组与加力组加力值差别大,而骨结合率却差别不大,说明骨结合率与力量关系不大,却与时间关系较大。(4)压力侧骨结合率略高于张力侧。 Objective Observe healing interface between rabbits bone and domestic-made micro-screws which immediate loading different force after 12 weeks, in order to study cohere sharp of bone interface and reconstruction around screws. Evaluate micro-screw''s stability and immediate loading capability, in order to further provide experimental evidence for clinical application of micro-screw in anchorage. Methods 60 micro-screws and 30 rabbits totally for experience, 2 screws every rabbit. Implant micro-screws, diameter 1.6mm and 9mm length, on diaphysis center area by micro-screw driver in local anesthesia. Every two screws draw reciprocal. 30 rabbits divided 3 groups random:0g(compare group),150g group and 300g group.Execute rabbits after 12 weeks, and get sample for sections. Get images for observe micro-screw''s healing interface. Measure bone contact ration of healing interface on every image. Results 1.The BCR of 150g group''s higher than compared group''s, and 300g group''s lower than compared group''s. All analysis showed that there was significant difference(P<0.01). 2.In the groups of immediately loading, BCR of pressure sides higher than tension sides have. The analysis of 150g group showed that there was not significant difference(P>0.05), and 300g group showed that there was difference(P<0.05). Conclusion 1.The Bone Implant Contact Ration of all experiment objects which immediately load force reach to 70% ~ 80%. It can keep stable anchorage for orthodontics clinical application when immediately loading force reach to 300g. 2.Larger force have certain impact in BCR of micro-screws. 3.Compared group and the loading group value big difference in force, but little difference in rate of BCR, indicating BCR and loading force has little, but the larger relationship with time. 4.The BCR of pressure sides slightly higher than tension sides have.
