摘要
目的 研究冠心病 (CHD)患者冠脉狭窄程度与 HDL 亚类组成的关系。方法 采用双相电泳 -免疫印迹法检测 5 1例 CHD患者及 5 6例正常人血清 preβ- HDL 及 α- HDL 各亚类的组成 ,并分析冠脉狭窄程度与各影响因素的相关关系。结果 CHD患者血清 HDL 颗粒有变小的趋势 ,并且随冠脉病变程度的增加 ,小颗粒的 preβ1 -HDL、HDL3b水平逐渐升高。而大颗粒的 HDL2 b水平却呈显著递减式减少 (P<0 .0 0 1 ) ,冠脉狭窄程度记分 (CSS)与血清中 preβ1 - HDL(P<0 .0 1 ) ,HDL3a(P<0 .0 5 ) ,HDL3b(P<0 .0 1 )呈正相关 ,与 HDL2 b(P<0 .0 1 )呈负相关。多元逐步回归分析结果显示 ,apo B1 0 0、HDL2 b、TG、HDL- C、TC/ HDL C、apo A 都是 CHD的独立危险因素。结论 CHD患者冠状动脉病变严重程度与 HDL
Objective To explore the relationship between the contents of serum HDL subclasses and the extent of coronary stenosis in coronary heart disease (CHD) patients. Methods The contents of serum HDL subclasses in CHD patients( n =51) and healthy controls ( n =56) were determined by two dimensional gel electrophoresis associated with immunodetection method. CHD patients were divided into three groups by average coronary severity score (CSS). Data were analyzed using linear regression and correlation method and multiple stepwise regression method. Results It was found that as the extent of coronary stenosis increases, the levels of pre β 1 HDL,HDL 3b increase, meanwhile the level of HDL 2b decreases ( P <0 001). There were significant positive correlations between CSS and pre β 1 HDL( P <0.01), HDL 3a ( P <0.05),HDL 3b ( P <0.01), and significant negative correlation was observed between CSS and HDL 2b ( P <0.01). apoB100,HDL 2b , TG, HDL C,TC/HDL C,apoAI are all independent risk factors of CHD. Conclusion The extent of coronary stenosis is highly correlated with the subclasses of HDL.
