nm23基因蛋白表达及临床病理因素对乳腺癌预后价值的多因素Cox回归分析 被引量:6

摘要 目的 :探讨乳腺癌组织中nm2 3基因蛋白表达及临床病理指标与预后的关系。方法 :应用免疫组化S P方法检测15 7例乳腺癌石蜡切片中nm2 3蛋白的表达情况 ;同时应用Kaplan Meier法及多变量Cox比例风险模型 ,分析nm2 3基因蛋白表达及临床病理因素与预后的关系。结果 :nm2 3基因蛋白表达率为 5 7 96% ,表达情况与肿瘤大小、TNM分期、腋淋巴结状态和远处转移相关 ;nm2 3蛋白表达阳性组患者生存率高于阴性组 ,P <0 0 1;多因素Cox回归分析显示 ,nm2 3蛋白表达、腋淋巴结转移情况及TNM分期 3项指标反映乳腺癌预后情况 ,危险度分别为 0 3 2 1、1 778及 1 0 0 5。结论 :nm2 3蛋白表达、腋淋巴结转移状况及TNM分期是乳腺癌的独立的预后指标 ,检测nm2 3有助于筛选出具有高复发及转移危险的患者 ,从而指导临床治疗。 OBJECTIVE:To determine the expression of nm23 and clini factor in relation to prognosis in breast carcinoma. METHODS:The expression of nm23 in 157 patients was determined by the S-P immunohistochemical method,and the Kaplan-Meier method and Cox proportional risk model were used to analyze the relationship of nm23 expression and clinicopathological factors with prognosis.RESULTS:The expression rate of nm23 was 57.96%,in connection with tumor size,TNM grade,the axillary lymph node status and distant metastasis.The results of survival analysis showed that longer overall survival time in the patients with nm23 positive expression than that in those without, P<0.01. In multivariate Cox regression analysis,the expression of nm23,the axillary lymph node status and TNM grade were three important prognostic factors of breast carcinoma. The risk ratio of nm23,the axillary lymph node status,and TNM grade was 0.321,1.778 and 1.005 respectively.CONCLUSIONS:In breast carcinoma,the expression of nm23,the axillary lymph node status and TNM grade may be an independent prognosticator,which would be helpful in selecting the high risk cases for further treatment.
