恶性实体瘤患者自体PHA-LAK细胞过继性免疫治疗对淋巴细胞表型及功能的影响 被引量:2

摘要 目的 :研究回输自体PHA LAK细胞对恶性实体瘤患者淋巴细胞表型及功能状态的影响。方法 :入组2 0 4例实体瘤患者 ,并以 10例健康成人作为对照 ,采集与分离外周血单个核细胞 ,体外扩增PHA LAK细胞 ,采用流式细胞术分析淋巴细胞表型 ,MTT法检测淋巴细胞杀伤活性。结果 :实体瘤患者较正常人淋巴细胞亚群CD3 + /CD4+ 细胞、CD3 + /CD8+ 细胞、B细胞、NK细胞减少 ,淋巴细胞杀伤活性下降。扩增的PHA LAK是以激活的淋巴细胞亚群CD3 + /CD8+ 细胞、CD3 + /CD56+ 细胞、CD4+ /CD2 8+ 细胞为主的异质性群体 ,其数量及杀伤活性明显增加 ,激活淋巴细胞标记性抗原分子表达亦增加。PHA LAK细胞回输治疗后 ,患者外周血上述激活淋巴细胞亚群数量增加 ,淋巴细胞杀伤活性增强。结论 :实体瘤患者淋巴细胞亚群数量及功能低下 ,PHA LAK细胞过继性免疫治疗使患者体内激活淋巴细胞亚群数量增加 。 Purpose:To investigate the phenotype and function of lymphocytes in patients with solid tumor as well as in healthy adults by autologous PHA-LAK cells from the patients. Methods:Peripheral blood mononuclear cells ( PBMC) from the 204 patients and the 10 healthy adult were incubated. The PBMC of patients were induced into PHA-LAK cells in vitro. The phenotype and the cytotoxicity of lymphocytes and PHA-LAK cells were analysed by flow cytometry and MTT method, respectively. Results:The subpopulation lymphocytes, mainly CD 3 +/ CD 4 +T cells,CD 3 +/ CD 8 +T cells,B cells,NK cells and the cytotoxicity of lymphocytes in patients with solid tumor were reduced compared with that in healthy adult. The PHA-LAK cells were shown to be a heterogeneous population with different cellular phenotypes. The percentage of activated lymphocytes CD 3 +/ CD 8 +T cells,CD 3 +/ CD 56 + cells,CD 4 +/ CD 28 + cells and the cytotoxicity of lymphocytes, were significantly increased at day 5~13 incubation. Some activated antigens such as CD 8 CD 28 CD 56 molecules in lymphocytes were over expressed. The subpopulation of activated lymphocytes, the activated antigens and the cytotoxicity of lymphocytes,were found to be enhanced after PHA-LAK cells adoptive therapy in the patients. Conclusions:The amount of subpopulation lymphocytes and the cytotoxicity of lymphocytes were found to be lower level in patients with solid tumor. PHA-LAK cells with activated lymphocytes population might been served as effective autologous adoptive immunotherapy for patients.
