摘要
目的:研究间质性肺疾病支气管肺泡灌洗液(BALF)中白细胞介素-6(IL-6)和粒细胞巨噬细胞集落刺激因子(GM-CSF)的水平及临床意义。方法:应用酶联免疫法(ELISA)检测12例结节病(Sar)及14例特发性肺纤维化(IPF)患者和10例健康人BALF及血清中IL-6和GM-CSF的水平,并与BALF的细胞学成分及临床数据进行相关分析。结果:Sar组和IPF组BALF中IL-6和GM-CSF水平均显著高于对照组(P<0.05,P<0.01),而血清中则无显著差别或测不到。IPF组BALF中IL-6与BALF中淋巴细胞百分比呈正相关,Sar与IPF组BALF中的GM-CSF则与BALF中肺泡巨噬细胞(AM)的百分比呈正相关(P<0.05,P<0.01)。结论:IL-6与GM-CSF在间质性肺疾病的发病过程中起一定作用。
To study the significance of IL-6 and GM-CSF in Pathological Mechanism of ILD. Methods:To measure the level of IL-6 and GM-CSF in sarcoidosis ,IPF and control groups, analyze the relation between the two cy-tokines and the cell constituents in BALF and clinical data. Results;①the levels of IL-6 in BALF of patients with sacoidosis and IPF increased significantly compared with control groups ( P <0. 05 ,P <0.01) ,but the levels in serum in three groups were with no statistical difference.②the levels of GM-CSF in BALF of patients with sacoidosis and IPF increased significantly compared with the control groups ( P <0. 05 ,P <0.01) ,but the levels in serum in three groups were indetectable. ③the levels of IL-6 in BALF of sarcoidosis and patients with sacoidosis and EPF were positively re-lated with lymphacyte percent in BALF, while the levels of GM-CSF in BALF of IPF were positively related with AM percent in BALF. There was no relation between the levels of IL-6 and GM-CSF in BALF and clinical data (such as sACE, lung function, activity of disease and the stage of the disease). Conclusion:①IL-6 and GM-CSF play an im-portant role in the pathological mechanism of lung fibrosis which could be through T lymphacyte or AM, direct action is not in exception.
