摘要
目的探讨锥状点式接触钢板与传统钢板治疗四肢长干骨骨折术后,骨愈合过程中X线像形态的差别,对比分析锥状点式接触钢板与传统钢板对骨折愈合的影响。方法回顾1994~2002年,我院随访资料完整的188侧长干骨骨折,其中121侧长干骨骨折用锥状点式接触钢板内固定术。67侧采用传统钢板内固定术。分别于术后6周、8周、3个月、半年,拆钢板前及拆钢板后各X线摄影一次,根据X线摄影观察骨痂分布、形成快慢、密度等情况,结合临床检查情况进行分析比较。结果用锥状点式接触钢板内固定术121侧,尺、桡骨双骨折9侧平均72.5±10.5天愈合,胫骨77侧平均愈合时间83.8±15.6天。股骨骨折35侧平均120.3±22.2天愈合。67侧采用传统钢板内固定术,尺、桡骨双骨折10侧平均85.5±13.5天愈合,胫骨27侧平均愈合时间110.3±20.1天,股骨骨折30侧平均152.4±30.1天愈合。两种治疗方法骨折愈合时间进行统计学分析(P<0.05)有显著性差异。结论锥状点式接触钢板能有效地保护钢板下皮质骨的血运,更有效的稳定骨折对位,其板下2mm间隙有利于血供重建,骨折愈合时间短,质量高,优于传统钢板内固定。
Objectives To discuss X-ray changes during healing process conical cylinder point contactfixation and conventional plate in the treatment of long bone fracture of limbs. To compare and analyze theeffect of the fracture healing between the two procedures.Methods 188 cases of long bone fracture during1994 and 2002 were reviewed. 121 cases were treated with conical cylinder point contact fixation, whereas67 cases were carried out with conventional plate. X-ray was taken at 6 weeks, 8 weeks, 3 months, 6 monthspost surgery, and before and after removal of the plate. The distribution, formation and density of calluswere observed. The results were compared and analyzed in conjunction with clinical situation. ResultsWithin 121 cases using conical cylinder point contact fixation, fracture of both ulna and radius, tibia,femur were 9, 77 and 35 cases respectively. Their average healing time was 72.5+10.5, 83.8+15.6 and 120.3+22.2 days. In the other 67 cases with conventional plate, fracture of both ulna and radius, tibia, femur were10, 27 and 30 cases respectively. Their healing time was 85.5+13.5, 110.3+20.1 and 152.4+30.1 days. Thedifference was statistically significant in the comparison of the healing time between two groups (t-testP<0.05). Conclusion The conical cylinder point contact fixation could protect the osteo-cortical circu-lation under the plate effectively. The 2mm space allowed the reestablishment of blood supply. The qualityand healing time of fracture were superior than the conventional plate.
