摘要
目的 探讨老年单纯收缩期高血压 (ISH)患者血清瘦素 (Lep)、抵抗素 (Res)与血浆同型半胱氨酸 (Hcy)、神经肽Y(NPY)的水平 ,及与其他类型老年高血压患者、健康老年人之间的差异。 方法 对ISH组 2 8例、全期型高血压 (SDH)组 2 6例、单纯舒张期高血压 (IDH)组 2 5例及老年健康对照组 2 1例 ,采用酶免疫法检测血清Lep、Res ,放射免疫法测定血浆Hcy、NPY ,同时检测受试者体质指数 (BMI)和体内脂肪 (BF)百分比。 结果 ISH组、SDH组、IDH组血清Lep〔(13 18± 1 6 6 )μg/L、(11 91± 2 16 ) μg/L、(10 88± 2 31) μg/L〕、Res〔(31 2± 10 3) μg/L、(2 6 3± 8 91) μg/L、(2 4 2± 5 6 6 ) μg/L〕水平均高于对照组〔(7 71± 1 2 8) μg/L和 (19 8± 7 2 1) μg/L ,P <0 0 5或P <0 0 1〕 ,ISH组显著高于SDH、IDH组 (P <0 0 5 ,P <0 0 1) ;3个不同类型高血压组患者的血浆Hcy、NPY较对照组增高 (P <0 0 1,P <0 0 5 ) ,而老年ISH组又较SDH、IDH组增高 (P <0 0 1) ;老年ISH组平均年龄较其他各组明显增高 (P <0 0 1) ,老年SDH组较IDH组、对照组也增高 (P <0 0 5 ,P <0 0 1) ;老年ISH组BMI也较其他各组明显增高 (P <0 0 5 ,P <0 0 1) ,而且SDH组较老年健康组显著增高(P <0 0 5 )。ISH?
Objective To study the levels of serum leptin, serum resistin, plasma cysteine and neuropeptide Y in the elderly patients of simple systolic hypertension, and the differences among the other types of the ederly hypertension and health elderly. Methods The radioimmunoassay method was used to determine the level of serum leptin, serum resistin, plasma cysteine (Hcy) and neuropeptide Y (NPY) for 28 elder ingenuous systolic hypertension patients (ISH), 25 ingenuous diastolic hypertension (IDH) patients, 26 systolic and diastolic hypertension (SDH) patients and 21 male healthy elders. Also the body weight index (BMI) and the percentage of fat distribution were detected. The difference and relationship were studied with statistical method. Results In the senile ISH group, the levels of serum leptin and serum resistin were significantly higher than those in the groups of SDH, IDH and normal controls(P<0.05, P <0.01), In the groups of IDH and SDH, the levels of serum leptin and serum resistin were also significantly higher than those in the normal controls. In the ISH group, the levels of Hcy and NPY were remarkably higher than those in the groups of IDH and SDH, and in all of the three hypertension groups, the level of plasma Hcy and NPY were higher than those in the normal controls (P<0.01, P<0.05). The average age in the ISH group was older than any other group, and average age of SDH group was older than that of the IDH group and the normal contro group (P <0.05, P <0.01). BMI of the ISH group was higher than that in any other groups ( P <0.05, P <0.01 ). There was linear relationship between the serum leptin and systolic blood pressure, as well as BMI (rwere 0.43 and 0.249 separately, P<0.01, r=0.386, P <0.01), as well as BMI (r=0.253, P <0.05). Linear relationship was also found between plasma NPY and systolic blood pressure(r=0.467, P <0.01). Conclusions There is possible influence of serum leptin, serum resistin, Hcy and NPY on the incidence of senile hypertension, especially on ISH. To control the levels of serum leptin, serum resistin, Hcy and NPY might be effective for the prevention and treatment of senile hypertension, especially ISH.
