摘要
目的:为开展腰椎间盘人工髓核假体(prostheticdiscnucleus,PDN)置换术提供影像解剖学资料。方法:对56例正常人腰椎间盘(男44例,女12例)MRIT2加权像中髓核矢状径、横径,椎间隙前、中、后高度进行测量。结果:髓核矢状径、横径及椎间隙高度均数之间差异显著性意义(t=0.84,P>0.05)。PDN假体前后径为12mm,本组髓核矢状径均>12mm,但>24mm者L2~323.2%,L3~421.4%,L4~521.4%,L5~S116.1%。PDN假体高度分别为5,7和9mm。椎间隙后高>5mm并且<9mm者L2~385.7%,L3~482.1%,L4~582.1%,L5~S158.9%,>9mm者L2~30%,L3~41.8%,L4~51.8%,L5~S13.6%,可选择相应的PDN高度型号。结论:大多数人腰椎间盘内仅可容纳单枚PDN假体,且适合高度为5mm或7mmPDN假体。
AIM:To provide the imaging anatomic data for replacement of the prosthetic disc nucleus(PDN). METHODS:The sagittal diameter and horizontal diameter of the nucleus pulposus,and the anterior,median and posterior heights of the intervertebral disc were measured in the lumbar MRI T2 images in 56 normal subjects(44 males and 12 females). RESULTS:No statistical difference was found in all measurements between males and females in the study(t =0.84,P >0.05).The sagittal diameters of the nucleus pulposus were >12 mm in all the lumbar levels,but the percentage of the sagittal diameter >24 mm was 23.2% at L2-3,21.4% at L3-4,21.4% at L4-5 and 16.1% at L5-S1,respectively.In addition,the percentage of the posterior height of intervertebral disc >5 mm and < 9 mm was 85.7% at L2-3,82.1% at L3-4,82.1% at L4-5,and 58.9% at L5-S1,respectively.The percentage of the posterior height >9 mm was 0% at L2-3,1.8% at L3-4,1.8% at L4-5,and 3.6% at L5-S1,respectively. CONCLUSION:One single PDN device is suitable for most patients and the commonly chosen height of the device is 5 mm or 7 mm of PDN.
