摘要
背景:大段同种异体骨移植逐渐应用于修复各种原因引起的骨缺损,由于影响异体骨移植愈合的因素较多,内固定因素对异体骨移植愈合影响目前研究较少。目的:研究内固定对异体骨段移植愈合影响的实验观察。设计:随机分组实验研究。地点和对象:实验在解放军第四军医大学西京医院骨科实验室完成,对象为新西兰大白兔36只,体质量2.2~3.4kg,年龄5~8月,雌雄不分,由第四军医大学实验动物中心提供。干预:采用新西兰大白兔股骨中段切除1.5cm骨干和骨膜实验动物模型。将36只兔随机分成圆形髓内针内固定组和三棱形髓内针内固定组。于术后第1,2,3月行ECT检查、苏木精-伊红(HE)染色组织学观察异体骨愈合过程。主要观察指标:移植骨段本身及周围组织病理变化和骨代谢情况。结果:圆形髓内针内固定组和三棱形髓内针内固定组异体骨移植愈合组织学过程相似,但三棱形髓内针内固定组在第1,2月时骨代谢活跃(t值均为2.286,P<0.05)。结论:稳固的髓内固定有利于异体骨愈合。
BACKGROUND:Large allograft bone transplantation is widely used in repairing bone defects caused by various reasons.There is very few researches on the effects of internal fixation on the healing of bone transplantation because of the diversified factors affecting healing. OBJECTIVE:To explore the effects of internal fixation on the healing of allograft bone transplantation. DESIGN:A randomized grouping experimental study was conducted. SETTING and PARTICIPANTS:The experiment was completed in the Laboratory of Department of Orthopaedics,Xijing Hospital,Fourth Military Medical University.Thirty white New Zealand rabbits aged from 5 to 8 months,weighing 2.2-3.4 kg were provided by Center for Experimental Animals,Fourth Military Medical University. INTERVENTIONS:1.5 cm middle shaft and periosteum of rabbit's femur were removed to make animal models.Thirty six rabbits were randomly divided into two groups,which were intramedullary fixation with circular pin group and intramedullary fixation with triangular pin group respectively.ECT and histological observation with HE staining were used to examine the healing process of allograft bone in the first,second and third month after operation. MAIN OUTCOME MEASURES:Histological and pathological changes of the allograft bone and the surrounding tissues as well as the bone metabolism. RESULTS:The histological process of circular pin group is very similar with that of the triangular pin group.However,the bone metabolism is more active in the first and second month in the triangular pin group(t=2.286,P< 0.05). CONCLUSION:Stable intramedullary fixation is good for the healing of allograft bone.
