摘要
目的 探讨注意缺陷多动障碍 (ADHD)患儿在脑干听觉反应 (ABR)检测中的特点。方法 应用美国NicoletSpirit脑电生理仪及Click短声刺激 ,测查 3 7例ADHD、2 4例精神发育迟滞 (MR)、2 2例品行障碍(CD)和 3 0名正常儿童 (NC)的ABR。结果 ADHD组、MR组、CD组及NC组在绝对潜伏期波Ⅲ、波Ⅴ (Fz脑区 ) ,波Ⅲ (Pz脑区 ) ,绝对波幅波Ⅲ (Fz脑区 ) ,波Ⅴ (Pz脑区 )上差异有显著性意义 (P <0 .0 5~ 0 .0 1)。在Fz脑区 ,波Ⅴ绝对潜伏期上与NC组相比 ,ADHD组、MR组和CD组均延迟 (P <0 .0 1) ;进一步分析MR又延迟于ADHD组和CD组。在Pz脑区上 ,ADHD组和MR组波Ⅲ绝对潜伏期均前移。绝对波幅上不论是波Ⅲ还是波Ⅴ ,ADHD组、MR组和CD组均低于NC组 (P <0 .0 1) ,进一步分析 ,在Pz脑区上 ,ADHD组和MR组波Ⅴ绝对波幅又明显低于CD组 (P <0 .0 1)。
Objective To investigate the features of brainstem auditoryresponses (BAR) in patients with attention deficit and hyperactive disorder (ADHD), mental retardation and conduct disorder. Methods BAR in children with ADHD ( n =37), mental retardation (MR, n =24) and conduct disorder (CD, n =22), and in normal children ( n =30) were measured by use of Nicolet Spirit electrophysiological instrument delivering stimulation of Clicks. Results The absolute latencies (AL) of wave Ⅲ、Ⅴ recorded from Fz region (WⅢ、Ⅴ/Fz) and wave Ⅲ from Pz region (WⅢ/Pz) as well as the absolute amplitudes (AA) of WⅢ/Fz and WⅤ/Pz were significantly different ( P <0.05) among ADHD, MR, CD, NC groups. Compared with those of NC group, the AL of WⅤ/Fz in ADHD, MR and CD groups were significantly delayed ( P <0.01). The AL of WV/Fz was delayed in ADHD, CD and MR children as compared to that of the NC( P <0.01), with that of the MR more significant. The AL of WⅢ/Pz in ADHD、CD groups were shortened. Compared with those in NC group, the AA of WⅢ and WⅤin ADHD, MP, CD groups were significantly decreased ( P <0.01). Further analysis showed that the AA of WⅤ/Pz in ADHD, MR groups were markedly decreased as compared to those in CD group ( P <0.01). Conclusion BAR could be of help in the clinical diagnosis of ADHD and MR.
