心脏不停跳心内直视手术的体外循环转流 被引量:2

摘要 目的 采用心脏不停跳、常温体外循环方法实施心脏直视手术,缩短手术时间,利于术后心功能恢复。方法 建立体外循环后,不阻断主动脉,不灌注心停跳液,心脏在窦性心律跳动下进行手术。结果 全部患者在转流过程中氧合效果良好,各项监测值均在正常范围,能顺利脱离体循环,术后心功能恢复好,痊愈出院。结论 常温体外循环方法,不用停跳和降温,维持冠状动脉的血流和氧供,避免因心脏停跳导致心肌缺血损伤及再灌注损伤,又可避免低温对心肌的损害,减少了并发症发生,利于术后心功能恢复。 Objective To evaluate the open-heart operation with beating-heart during normothernic cardiopulmonary bypass. Methods After the cardipulmonary bypass was built, the heart was operated under sinus rhythm beating in the condition that the aorta was not crossclamped and the cardioplegic solution was not perfused. Results: The oxygenation effect of all patients was good in the crossover bypass. All monitoring values were within normal range, the patients were divorced from cardiopulmonary bypass and the heart function recovery was good. All the patients were cured. Conclusion The normothermic cardiopulmonary bypass can keep the bolld flow and oxygen delivery of coronary artery without heart arrest and cooling, avoid myocardial ischemia and reperfusion injury because of heart arrest and the myocardial damage because of cooling, reduce the occurrence of the complication and it is helpful for heart function recovery.
