摘要
目的 探讨Leber遗传性视神经病变 (Leber′shereditaryopticneuropathy ,LHON)的临床治疗方法及效果。方法 观察 2 8例LHON患者 ,其中 2 2例DNA检测阳性 ,6例DNA检测阴性者有明确的家族遗传史和典型的临床特点。所有患者接受了神经营养和保护剂的治疗 ,包括中药血管扩张剂或微循环改善剂、针灸以及复方樟柳碱颞侧皮下注射等综合治疗。治疗和观察时间平均为 13 6天。结果 2 8例LHON患者的 5 6只眼经综合治疗后 2 6只眼视力有所提高 ,治疗总有效率为 4 6 5 %。其中视力≥ 0 0 5者 9只眼 ,占 16 % ,6只眼的视力恢复至 0 4 ,占 11%。 30只眼视力无改善 ,占 5 3 6 %。结论 DNA的检测有助于LHON患者及家系的诊断、治疗及遗传咨询。综合疗法对部分患者显示一定的治疗效果 ,LHON的治疗概念将会被重新认识。
OBJECTIVE To discussed clinically therapeutic approaches and effect on Leber′s hereditary optic neuropathy(LHON) METHODS Twenty-eighty patients with LHON were observed and treated There were 22 positive results among the 28 patients when their DNA was examined Therewere specific family history and clinical characteristics in 6 patients whom DNA was negative The all patients were treated with nutrients and protective of nerve,including the Chinese medicine-vascular expansion,temporal hypodermic injection of compound anisodine Average of period of treatment was 13 6 days RESULTS After the overall treatment visual acuity of 26 eyes was improvement in 56 eyes of the 28 patients with LHON Therapeutic efficiency was 46 5% Visual acuity was 0 05 or more in 9 eyes(16%),and was 0 4 or more in 6 eyes(11%) Visual acuity was not improvement in 30 eyes(53 6%) CONCLUSION The therapeutic concept of the blindness-causing optic disease will be re-considered The overall treatment on some patients demonstrates certain effect and the DNA test is helpful for the diagnosis,treatment and hereditary consultation of the patients with LHON and their family clans
