摘要 目的:探讨Bell麻痹患者行瞬目反射(blinkreflex,BR)、面神经电图(electroneurogam,ENoG)对面神经损害程度及部位的评估价值。方法:20例Bell麻痹患者在发病后1~14d分别行BR,ENoG检查。结果:患侧R1,R2及R2’波缺如者5例,余15例患侧较健侧R1,R2及R2’波潜伏期延长,异常率100%。患侧面神经运动潜伏期延长,波幅低,与健侧比较差异有显著性意义(P<0.01)根据R1/M潜伏期比值测定结果提示,10例为近端(茎乳孔内段)损害,5例为远端(茎乳孔外段)损害,5例为较大范围受累。结论:BR检测是诊断Bell麻痹的早期敏感指标,而采用BR,ENoG综合检测Bell麻痹的患者,可全面、客观地评价面神经损害的程度及部位,对临床治疗和预后评估有一定的帮助。 AIM: To explore the value of blink reflex(BR) and facial electroneurogam (ENoG ) in evaluating the degree and location of facial nerve damage in patients with Bell palsy. METHODS:BR and ENoG were conducted in 20 patients with Bell paralysis respecti vely 1-14 days after episode. RESULTS: The ipsilateral R1,R2 and R2' waves were absent in 5 cases,and in th e remaining 15 cases,the latencies of R1,R2 and R2' were prolonged in the ipsil ateral side as compared with those in the uninjured side, and the abnormal rate was 100%.There were longer latent period and lower wave amplitude of facial ner ve in the ipsilateral side,significantly different from those in the uninjured s ide(P< 0.01).The ratio of R1 /M lateral indicated that there were 10 cases of pr oximal(inside of stylomastoid foramen) injury,5 cases with distal(outside of sty lomastoid foramen) injury and 5 cases with extensive injury. CONCLUSION:BR can be used as a sensitive index for early diagnosis of Bell par alysis.BR and ENoG can evaluate the degrees and location of facial nerve damage wholly and objectively in patients with Bell paralysis,and it is helpful for the clinical treatment and prognostic evaluation.
