摘要 目的:探讨甘油果糖对颅脑损伤后患者血液流变学的影响,揭示其降低颅内高压的作用机制。方法:采用前瞻性临床对照研究,将颅脑损伤患者随机分组为:实验组(30例)和对照组(30例),分别给予甘油果糖和甘露醇治疗,对两组患者治疗前后的血液流变学指标作比较。结果:实验组血沉由治疗前(47±9)mm/h降至治疗后第7天(22±4)mm/h,对照组由治疗前(47±8)mm/h到治疗后第7天(24±6)mm/h,治疗前后相比差异均有非常显著性意义(P<0.01),但实验组疗效更显著(P<0.05)。其他的血液流变学指标(血细胞比容、全血比黏度、全血还原比黏度、血浆比黏度、血细胞聚集指数)实验组均较对照组好,差异有显著性意义(P<0.05或0.01)。结论:甘油果糖能有效改善脑损伤后的血液流变学状态,对改善微循环、减轻继发性脑水肿和降低颅内压有重要的临床意义。 AIM:To explore the influence of glycerol fructose on hemorrheologic features o f patients following brain injury,so as to reveal its mechanism to reduce acute intracranial hypertension. METHODS:In the prospective clinical controlled study,patients with head injury were divided randomly into experimental group(n=30,treated with glycerol fructo se) and control group(n=30,treated with mannitol).The hemorrheologic indexes bef ore and after treatment were compared between the two groups. RESULTS:The blood sedimentation in the experimental group was decreased to(22 ±4) mm per hour on the 7th day from(47±9) mm per hour before treatment,and t hat in the control group was decrease to(24±6) mm per hour on the 7th day from( 47±8) mm per hour before treatment,and there were very significant differences compared with those before treatment(P< 0.01),while the efficacy was more signif icant in the experimental group(P < 0.05).Other hemorrheologic indexes,including hematocrit,blood viscosities,blood reduction viscosities,plasma viscosities and erythrocyte aggregation index in the experimental group,were markedly better in the experimental group than those in the control group(P< 0.05 or 0.01). CONCLUSION:Glycerol fructose can effectively ameliorate the hemorrheologic sit uations after brain injury,which is important to improve microcirculation,reliev e secondary brain edema and reduce the intracranial pressure in clinic.
