摘要
目的 研究不同新辅助化疗方案能否引起进展期大肠癌肿瘤细胞的凋亡以及对大肠癌肿瘤细胞增殖细胞核抗原 (Proliferatingcellnuclearantigen ,PCNA)表达的影响。方法 应用原位末端标记 (Terminaldeoxynucleotidyltransferasemediatednickendlabeling ,TUNEL)和直接免疫荧光法检测细胞凋亡 ,采用链菌亲和素 生物素 过氧化物酶复合物 (Avidinorstreptavidin biotin peroxidasecomplex ,ABC)标记的免疫组化法检测大肠癌组织的PCNA表达 ,对 96例大肠癌患者组织中的正常大肠黏膜和肿瘤细胞的凋亡指数 (Apoptotisindex ,AI)和增殖指数 (Proliferativeindex ,PI)分别进行检测。结果 两个新辅助化疗组患者肿瘤细胞AI分别为 7.4 7%和 6 .14 % ,与手术对照组肿瘤细胞AI 4 .83%相比明显增高 (P <0 .0 1) ,与单纯对照组大肠黏膜细胞AI2 .74 %相比明显增高 (P <0 .0 1) ;两个新辅助化疗组中患者肿瘤细胞PI为分别 33.71%和 39.6 8% ,与手术对照组肿瘤细胞PI5 1.5 1%相比明显降低 (P <0 .0 1) ,与单纯对照组大肠黏膜细胞PI 19.82 %相比明显增高 (P <0 .0 1)。两个新辅助化疗组间AI与PI相比具有显著差异 (P <0 .0 1)。结论 新辅助化疗能诱导肿瘤细胞发生凋亡 ,并能抑制其增殖 。
Objective To investigate the effects of two different protocols of neoadjuvant chemotherapy on cell apoptosis and expression of proliferating cell nuclear antigen(PCNA) in progressing colorectal carcinoma. Methods Apoptosis index (AI) of tumor cells was assayed by direct immunofluorescence and terminal deoxynucleotidyl transferase mediated nick end labeling (TUNEL),whereas proliferation index(PI) was examined by immunohistochemical labelling of avidin or streptavidin-biotin-peroxidase complex(ABC ) in 96 patients with colorectal carcinoma. Results Tumor cell AI in the two neoadjuvant chemotherapy groups were 7.47% and 6.14%,significantly higher than 4.83% in the operative control group(P<0.01), and also significantly higher than 2.74% in the control group with normal colorectal mucosa (P<0.01).The PI positive expression rates in the two neoadjuvant chemotherapy groups were 33.71% and 39.68%,significantly lower than 51.51% in operative control group(P<0.01),and significantly higher than 19.82% in the normal control group(P<0.01).There was significant negative correlation between AI and PI in the two neoadjuvant chemotherapy groups(P<0.01). Conclusions Neoadjuvant chemotherapy induces tumor cell apoptosis and inhibits tumor cell proliferation in human colorectal carcinoma. The extent of imbalance between colorectal carcinoma cell proliferation and apoptosis is correlated with different protocols of neoadjuvant chemotherapy.
