nm23基因在乳腺癌的表达及其临床价值

摘要 目的 探讨nm2 3表达水平对判断乳腺癌生物学行为和预后的临床价值。方法 采用免疫组织化学S P法 ,检测石腊包埋 4 2例乳腺癌组织和 4 0例乳腺囊性增生症中nm2 3蛋白的表达水平。结果 nm2 3表达与年龄、肿瘤大小、绝经前后、ER、术后复发、组织分级和临床分期无相关性 ;nm2 3高表达与腋淋巴结转移情况有显著负相关性 ,nm2 3高表达与患者远处转移有显著负相关性 ,与生存率呈正相关。全组 5年生存率为 6 4 30 %。在腋淋巴结阴性、nm2 3高表达的 15例中 ,5年生存率达86 6 7% (13/ 15例 ) ,仅 4例出现术后远处转移 ,3例出现术后复发。结论 检测nm2 3的表达可作为乳腺癌生物学行为和预后的指标。 Objective To investigate the clinical value of expression of nm23 gene in predicting biological behaviors and prognosis of breast Cancer.Methods Immunohistochemistry (S-P method) was used to detect the expression of nm23 gene in forty-two samples of paraffin embedded human breast cancer tissues, and forty cases of paraffin embedded hyperplasia of mammary glands tissues. All the slides were scored semi-quantitatively based on staining intensity and distribution.Results The expression of nm23 did not correlated with age, tumor size, estrogen receptor, disease recurrence, clinical stage, and histological grade. The high expression of nm23 was significantly inversely correlated with axillary lymph node metastasis, P<0.001, and was in positive relationship with the survival rate of 5-year, P=0.038. The high expression of nm23 in breast cancer was negatively correlated with distant metastasis, P<0.001. In the all patients, the 5-year survival rate was 64.30%. In the 15 patients with the negative axillary lymph node metastasis and high expression of nm23, the 5-year survival rate was 86.67%(13/15 cases), only four cases had distant metastasis, and three had disease recurrence. But in the 13 patients with the positive axillary lymph node metastasis and with the low expression of nm23, the 5-year survival rate was 30.77%(4/13 cases, P=0.003), eleven cases had distant metastasis (P=0.002), and three had disease recurrence (P=0.024).Conclusion This study suggested that examination of expression of nm23 gene might be a predictor for biological behaviors and prognosis of breast cancer.
