摘要
目的 探讨海水浸泡烧伤复合破片伤伤后火器伤伤道组织病理学变化与细菌学变化之间的关系。方法 成功建立复合伤模型后 ,依模型将 15只成年杂种犬随机分为单纯复合伤组 (简称非浸泡组 7只 )和海水浸泡复合伤组 (简称浸泡组 8只 ) ,于伤后即刻、4、7、10、2 0、2 8小时共 6个时相点 ,用微穿刺针穿刺活检取肌组织行细菌培养 (距伤道 1.5cm取材 ) ,另于伤后 4、2 0、2 8小时时相点取距伤道 0 .5、1.5cm微穿刺针取骨骼肌组织作病理学检查。结果 海水浸泡复合伤动物火器伤局部组织感染出现早 ,感染严重 ;同时与之相伴的是局部组织坏死加剧 ,坏死组织伴有明显脱水。结论 海水浸泡明显加重复合伤伤道局部组织的损伤 ,浸泡后伤道组织感染的严重程度与海水浸泡后导致的组织坏死的严重程度有密切关系。
Objective To observe the relationship between histopathology and bacteriology changes of projectile-burn combined wound in dogs with seawater immersion.Methods After creation model of projectile-burn combined wound in dogs with seawater immersion,15 dogs were randomized into 2 groups:projectile-burn-only(non-immersion group,UI),projectile-burn combined wound with seawater immersion(immersion group,I).Small pieces of muscle tissue samples were taken for bacterial culture after injury immediately and 4,7,10,20,28h later (samples were obtained from 1.5cm points distance from local wound tract by little acpuncturer).Small pieces of local fire-arm injury tissue samples were taken for examination with light microscope(LM)after injury,and 4,20,28h later(samples getting from 0.5,1.5cm points distance from local wound tract).Results Local wound tract tissue was found to be severely necrotic in immersion group,and the necrotic tissue had dehydrated obviously.Local tissue had earlier and more severe infection.Conclusion Seawater immersion aggravates damage in local fire-arm injury tissue of animals with burn-projectile combined wound.There is a close relationship between the degree of local tissue infection and necrosis.
